消毒供应中心医疗器械清洗消毒效果影响因素的研究
Risk Factors of the Cleaning and Disinfection Effect of Medical Instruments in Central Sterile Supply Department
【摘要】 目的:了解消毒供应中心(CSSD)医疗器械清洗消毒效果影响因素,以进一步规范CSSD管理,保证医疗器械清洗消毒质量,为预防和控制医院感染提供参考依据。方法:检索2010年1月~2022年5月CSSD清洗消毒相关文献,汇总文献中所有影响医疗器械清洗消毒效果的因素,制作成调查问卷。通过深入访谈的方式听取专家意见,对问卷中影响因素进行修改,再运用德尔菲法对专家进行咨询,确定最终CSSD医疗器械清洗消毒效果影响因素。结果:咨询专家的权威系数为0.88%,经过两轮调查共确定包括使用后未及时去除明显污染物,使用后未根据需要及时保湿处理,污染器械未及时转运至CSSD等,26条日常工作中常见影响因素,平均分均>4.0,标准差>0.4,变异系数均<0.25。结论:本研究运用德尔菲法最终确定六大环节26条影响医疗器械清洗消毒效果的因素,提示医疗器械在清洗消毒环节受到多种因素的影响。针对性采取应对干预措施,能够保障医疗器械清洗消毒效果,减少产生不良影响。
【Abstract】 Objective: Explore the risk factors affecting the cleaning and disinfection effect of medical devices in central sterile supply department(CSSD), further standardize the management of CSSD, ensure the cleaning and disinfection quality of medical devices, and provide reference basis for the prevention and control of hospital infection. Methods: Retrieve CSSD cleaning and disinfection related documents from January 2010 to May 2022, summarize all factors affecting the cleaning and disinfection effect of medical devices in the documents, and prepare a questionnaire. Listen to the experts’ opinions through in-depth interviews to modify the influencing factors in the questionnaire, and then use Delphi method to consult the experts to determine the final influencing factors of CSSD medical device cleaning and disinfection effect. Results: Retrieve CSSD cleaning and disinfection related documents from January 2010 to May 2022, summarize all factors affecting the cleaning and disinfection effect of medical devices in the documents, and prepare a questionnaire. Listen to the experts’ opinions through in-depth interviews to modify the influencing factors in the questionnaire, and then use Delphi method to consult the experts to determine the final influencing factors of CSSD medical device cleaning and disinfection effect. The authority coeficient of the consulting expert is 0.88%. After two rounds of investigation, it is determined that there are 26 common influencing factors in daily work, including failure to remove obvious pollutants in time after use, failure to moisturize as needed after use, and failure to transport contaminated equipment to CSSD in time. The average score is >4.0, the standard deviation is >0.4, and the coeficient of variation is <0.25. Conclusion: The study utilized the Delphi method to ultimately identify six major aspects and 26 factors that affect the effectiveness of medical device cleaning and disinfection, indicating that the cleaning and disinfection process of medical devices is influenced by a variety of factors. Targeted intervention measures can ensure the cleaning and disinfection effect of medical devices and avoid adverse effects.
