胎儿端脐动脉VTI联合胎盘端脐静脉TAmax预测低出生体质量
Combined Fetal End-Umbilical Artery VTI with Placental End-Umbilical Vein TAmax in Prediction of Low Birth Weight
【摘要】 目的 探讨胎儿端脐动脉速度时间积分(VTI)联合胎盘端脐静脉时间平均峰值流速(TAmax)预测低出生体质量(LBW)的价值。方法 收集产检并最终住院生产的393例单胎孕妇资料,包括孕妇基本情况、血生化指标、产前超声检查各项参数。统计分析相关数据并绘制ROC曲线,评价胎儿端脐动脉VTI联合胎盘端脐静脉TAmax预测LBW可行性。结果 (1)多因素回归分析显示:胎儿端脐动脉VTI(OR:0.980,95%CI:0.965~0.995,P=0.009)、胎盘端脐静脉TAmax(OR:0.826,95%CI:0.691~0.986,P=0.034)是LBW的独立影响因素。(2)联合胎儿端脐动脉VTI与胎盘端脐静脉TAmax预测LBW,拟合回归方程Y=4.019-0.020×胎儿端脐动脉VTI-0.192×胎盘端脐静脉TAmax,其联合指标Y≥-0.601,预测LBW的ROC曲线下面积为0.777,标准误为0.044,P<0.001,灵敏度为68.75%,特异度为80.61%,约登指数为0.49;以胎儿端脐动脉VTI≤117.5 cm或胎盘端脐静脉TAmax≤15.72 cm/s其中之一为预测指标,其灵敏度为81.25%,特异度为60.61%,约登指数为0.42。结论 胎儿端脐动脉VTI、胎盘端脐静脉TAmax是LBW的独立影响因素,联合此两指标可预测LBW,具有一定的临床应用价值。
【Abstract】 Objective This study aimed to evaluate the value of fetal end-umbilical artery velocity-time integral(VTI) combined with placental end-umbilical vein time-averaged maximum velocity(TAmax) in predicting low birth weight(LBW). Methods The data of 393 single pregnant women who underwent prenatal ultrasonic examination and gave birth in our hospital were collected, including the basic conditions of pregnant women, blood biochemical indexes and parameters of prenatal ultrasonography. Relevant data were statistically analyzed and ROC curves were drawn to evaluate the feasibility of fetal end-umbilical artery VTI combined with placental end-umbilical vein TAmax in predicting LBW. Results(1)Multivariate regression analysis showed that fetal end-umbilical artery VTI(OR:0.980, 95%CI:0.965-0.995,P=0.009) and placental end-umbilical vein TAmax(OR:0.826, 95%CI:0.691-0.986,P=0.034) were independent influencing factors of LBW.(2)The combination of fetal end-umbilical artery VTI and placental end-umbilical vein TAmax to predict LBW, the regression curve was fitted as Y=4.019-0.020× fetal end-umbilical artery VTI-0.192× placental end-umbilical vein TAmax. When the combined index Y≥-0.601, the area under the ROC curve of predicted LBW was 0.777 with a standard error of 0.044, P<0.001, and with a sensitivity of 68.75%, a specificity of 80.61%, a Youden index of 0.49. Selected one of fetal end-umbilical artery VTI≤117.5 cm or placental umbilical vein TAmax≤15.72 cm/s as predictive index, that the sensitivity, specificity and Youden index were 81.25%, 60.61% and 0.42. Conclusions Fetal end-umbilical artery VTI and placental end-umbilical vein TAmax are independent influencing factors of LBW, and the combination of the two indicators can predict LBW, which have certain clinical application value.
