颌下腺原发性非霍奇金淋巴瘤的超声表现

【摘要】 目的 分析颌下腺原发性非霍奇金淋巴瘤(PSGNHL)的声像特征,提高超声诊断水平。方法 结合临床与病理资料,回顾性分析13例PSGNHL患者的超声表现。结果 12例为肿块型:多表现为实性(83.33%)、极低回声(83.33%)、形态不规则(66.67%)、边缘不规则或模糊(66.67%)、内部回声不均匀(66.67%)、后方回声增强(100%)、内部无钙化(100%)、血流丰富(91.67%)、周边腺体有受压且回声不均(75.00%)。1例为弥漫型:表现为实性、等回声、边缘不规则、内部回声不均匀、内部无钙化、血流丰富。结论 PSGNHL声像表现多为肿块型、实性、极低回声、形态不规则、边缘不规则或模糊、内部回声不均匀、后方回声增强、内部无钙化、血流丰富、周边腺体有受压且回声不均匀;好发于中老年女性,临床多表现为无痛性肿块;掌握以上特征有助于正确诊断PSGNHL。

【Abstract】 Objective To investigate the ultrasonographic characteristics of primary submandibular gland non-Hodgkin lymphoma(PSGNHL), and improve the diagnostic ability in this disease with ultrasonography. Methods Combined with clinical and pathological data, the ultrasonic manifestations of 13 cases of PSGNHL were retrospectively analyzed. Results 12 cases were mass type: mostly solid(83.33%), very low echo(83.33%), irregular shape(66.67%), irregular or blurred boundary(66.67%), uneven internal echo(66.67%), and enhanced posterior echo(100%). There was no internal calcification(100%), rich blood flow(91.67%), the peripheral glands were compressed and the echo was uneven(75.00%). 1 case was diffuse: solid, isoechoic, border lobed or irregular, internal echo uneven, no internal calcification, rich blood flow. Conclusions The ultrasonographic characteristics of PSGNHL are mostly mass type, solid, very low echo mass, irregular shape, irregular or blurred boundary, uneven internal echo, enhanced posterior echo, no internal calcification, rich blood flow, peripheral glands compression and uneven echo, more prevalent among middle-aged and elderly women, and the clinical manifestations are mostly painless masses. The above characteristics are helpful to diagnose PSGNHL.
