基于头颈CTA的牙周炎与动脉粥样硬化相关性研究
Study on the Correlation Between Periodontitis and Atherosclerosis Based on CTA of Head and Neck
【摘要】 目的 基于头颈CTA检查探讨牙周炎严重程度与头颈动脉粥样硬化的关系,对脑血管病预防提供参考。方法 收集某医院2022年1月1日-2023年7月31日行头颈CTA检查、并于一周内在口腔科做牙周检查的87例患者,根据牙周炎诊断标准分为四组:无牙周炎组(19例),轻度牙周炎组(24例),中度牙周炎组(23例),重度牙周炎组(21例)。详细记录并比较四组病例性别、年龄、头颈动脉不同程度狭窄、狭窄血管段数及斑块类型有无差别。结果 四组病例的性别、年龄没有统计学差异(P>0.05)。有牙周炎组血管狭窄占比(95.8%~100%)明显高于无牙周炎组(42.1%);牙周炎程度越重,血管狭窄占比越高,狭窄程度越重(P<0.05)。有牙周炎组狭窄血管段数(96~170段)明显多于无牙周炎组(26段);牙周炎程度越重,狭窄血管段数越多(P<0.05)。四组病例血管斑块类型无统计学差异(P均>0.05);斑块总数有统计学差异(49~226枚),牙周炎程度越重,斑块数量越多(P<0.05)。结论 牙周炎和头颈动脉粥样硬化密切相关,积极治疗牙周炎对预防脑血管病有重要临床意义。
【Abstract】 Objectives This study aims to investigate the relationship between the severity of periodontitis and atherosclerosis of head and neck based on CTA examination, so as to provide a reference for the prevention of cerebrovascular disease. Methods A total of 87 patients who underwent head and neck CTA examinations at a hospital from January 1, 2022, to July 31, 2023, and underwent periodontal examinations within one week were included in the study. According to the diagnostic criteria of periodontitis, the 87 patients were divided into four groups: no periodontitis group(19 cases), mild periodontitis group(24 cases), moderate periodontitis group(23cases) and severe periodontitis group(21 cases). The sex, age, different degree of head and carotid artery stenosis, number of stenosis segments and plaque type of the four cases were recorded and compared in detail.Results There were no significant differences in gender and age among the four groups(P>0.05). The proportion of vascular stenosis in the group with periodontitis(95.8%~100%) was higher than that in the group without periodontitis(42.1%). The severity of periodontitis correlates positively with both the proportion and severity of vascular stenosis, with statistical significance(P<0.05). The number of narrow vascular segments in the group with periodontitis(96~170 segments) was significantly more than that in the group without periodontitis(26segments). There is a positive correlation between the severity of periodontitis and the number of narrow vascular segments(P<0.05). There was no significant difference in the types of vascular plaques among the four groups(P>0.05). There was a statistically significant difference in the total number of plaques(range: 49 to 226), with a positive correlation observed between the severity of periodontitis and the number of plaques(P<0.05).Conclusions There is a close relationship between periodontitis and the degree of head and carotid artery lesions.Active treatment of periodontitis has important clinical significance in preventing cerebrovascular disease.
