基于“脾为五脏之使”探讨肺结节“结-癌”转化及防治思路

【摘要】 “脾为五脏之使”是对脾为他脏提供水谷精微及脾是他脏之气周流中枢的高度概括。基于此理论,脾土虚弱、气机失调是肺结节“结-癌”转化的核心,脾虚气弱,他脏失养,气运无力,聚湿成痰,气停血滞,痰湿瘀血,阻于肺络,发为结节,胶结日久,郁而化热,酿成癌毒,终成肺癌。因此,治疗中应将补脾调气贯穿始终,结合各阶段特点灵活运用祛湿、化痰、行气、活血、清热、解毒、散结等法,以期减缓或截断转化进程。

【Abstract】 “The spleen is the helper of the five zang organs” is a high summary of the fact that the spleen provides the nutrition for the other zang organs and that the spleen is the center of qi circulation of the other zang organs. Based on this theory, the weakness of spleen and the imbalance of qi circulation are the core of the transformation of “nodule-cancer”of pulmonary nodules. Spleen deficiency and qi weakness, loss of nourishment of the other zang organs, weakness of qi movement, accumulation of dampness into phlegm, qi stagnation and blood stasis, phlegm and blood stasis, obstruction in lung collaterals, turning into nodules, cementation for a long time, depression turns into heat, turning into cancer toxin and lung cancer eventually. Therefore, tonifying spleen and regulating qi should be carried through all the time, and the methods of removing dampness, resolving phlegm, activating qi, activating blood circulation, clearing heat, detoxification, and dispersing nodule should be flexibly used in the treatment combined with the characteristics of each stage, so as to slow down or cut off the process of nodule-cancer transformation of pulmonary nodules.
