【摘要】 目的 探讨非特殊型浸润性乳腺癌(invasive breast carcinoma of no specific type, IBC-NST)边缘带二维超声特征、超声造影增强特征及定量参数与Ki-67的相关性。方法 收集2019年10月～2023年10月在山西医科大学第一医院经手术病理及免疫组化证实的IBC-NST患者84例(84个单侧单发病灶),年龄25～78岁，所有患者术前均未进行放化疗，且均行二维超声、超声造影检查。根据Ki-67表达程度将其分为高表达组和低表达组，分析对比两组病灶边缘带二维超声特征、超声造影增强特征及定量参数，分析定量参数与Ki-67的相关性。结果 Ki-67高表达组IBC-NST患者病灶边缘带边界模糊比例高于Ki-67低表达组，差异均有统计学意义(P<0.05)。Ki-67高表达组IBC-NST患者病灶边缘带增强特征、边缘放射状增强、造影后范围扩大比例均高于Ki-67低表达组，差异均有统计学意义(P<0.05)。Ki-67高表达组IBC-NST患者病灶边缘带超声造影定量参数峰值强度(peak intensity, PI)、曲线下面积(area under the curve, AUC)均高于Ki-67低表达组，差异均有统计学意义(P<0.05)。IBC-NST边缘带定量参数PI、AUC与Ki-67表达呈正相关，差异均有统计学意义(P<0.05)。结论 IBC-NST患者病灶边缘带边界模糊、边缘放射状增强、造影后范围扩大、定量参数PI、AUC升高可提示Ki-67高表达，为临床个体化精准治疗提供了一定的影像学指导。更多还原

【Abstract】 Objective To explore the correlation between marginal zone two-dimensional ultrasound features, enhancement features and quantitative parameters of contrast-enhanced ultrasound and Ki-67 in the invasive breast carcinoma of no specific type(IBC-NST). Methods From October 2019 to October 2023, 84 patients with IBC-NST(84 unilateral lesions), aged 25 to 78 years, confirmed by surgical pathology and immunohistochemistry were collected from the First Hospital of Shanxi Medical University. All patients had not received preoperative chemoradiotherapy, and underwented two-dimensional ultrasound and contrast-enhanced ultrasound examination. They were divided into high expression group and low expression group according to the expression degree of Ki-67. The two-dimensional ultrasound features and contrast-enhanced ultrasound features and quantitative parameters of two groups were analyzed and compared, and their correlation between the quantitative parameters and Ki-67 was analyzed. Results The proportion of margin indistinct of IBC-NST lesion in Ki-67 high expression group was higher than that in Ki-67 low expression group, and the differences were statistically significant(P<0.05). The proportion of adial enhancemen and the range enlargement after angiography of IBC-NST lesion in Ki-67 high expression group was higher than those in Ki-67 low expression group, and the differences were statistically significance(P<0.05). The quantitative parameters peak intensity(PI), area under the curve(AUC) of IBC-NST lesion in Ki-67 high expression group were higher than those in Ki-67 low expression group, and the differences were statistically significant(P<0.05). IBC-NST marginal zone quantitative parameters PI, AUC were positively correlated with Ki-67 expression, and the differences were statistically significant(P<0.05). Conclusion In patients with IBC-NST, the marginal zone of the lesion is margin indistinct, the radial enhancement, the srange enlargement after angiography, and the quantitative parameters PI, AUC are increased, indicating the high expression of Ki-67, providing certain imaging guidance for clinical individualized precision treatment.更多还原
