复发性髌骨脱位危险因素的研究进展
Research progress on risk factors of recurrent patellar dislocation
【摘要】 髌骨脱位是儿童和青少年中最常见的膝关节损伤之一,在首次发生髌骨脱位后,反复脱位的风险仍然相当高,并容易导致膝关节活动度减少、加速膝关节骨关节炎的恶化。但是目前关于哪些患者首次脱位容易出现髌骨不稳甚至反复脱位尚不明确,归纳复发性脱位患者的危险因素,利用预测模型进行分类管理对临床干预是非常有意义的。本综述将分析复发性髌骨脱位患者的解剖因素和人口统计学特征,探究危险因素的综合效应,讨论用于评估复发性髌骨脱位的预测模型的作用,旨在识别复发性髌骨脱位的高危人群,为临床个体化评估和治疗提供依据。
【Abstract】 Patellar dislocation is one of the most common knee injuries in children and adolescents. The risk of recurrent patellar dislocation remains high after the first dislocation, that can lead to reduced knee mobility and accelerated progression of knee osteoarthritis. At present, it is not clear which patients are prone to patellar instability or even repeated dislocation in the first dislocation. Therefore, generalisation of risk factors in patients with recurrent dislocations and categorical management using predictive models is necessary for clinical intervention. This review will analyzing anatomical and demographic factors contributing to recurrent patellar dislocation, explore the combined effects of risk factors and discuss the role of predictive models for recurrent instability and scoring. The aim is to identify those at high risk of recurrent patellar dislocation and to provide a basis for individualised clinical assessment and treatment.
