窄带成像技术不同分型诊断声带白斑良恶性的价值

【摘要】 目的 探讨电子喉镜下白光模式及窄带成像(NBI)模式分型鉴别诊断声带白斑(VCL)性质的价值。方法 136例VCL患者均接受白光模式、 NBI模式检查,比较不同病变性质患者的病理特征,分析不同分型诊断VCL性质的价值。结果 良性组的吸烟、病变累及前联合、累及范围≥50%占比均低于恶性组(P <0.05)。NBI模式的NI分型诊断VCL病变性质的准确度最高,其次为白光模式下的CHEN分型;NBI模式的NI分型鉴别VCL性质的Kappa值最高。结论 电子喉镜下白光模式及NBI模式对VCL性质鉴别均具有一定的价值,其中NBI模式下NI分型与病理检查的一致性最高。

【Abstract】 Objective To investigate the value of types of white light mode and narrow band imaging(NBI) mode under electronic laryngoscope in differential diagnosis of properties of vocal cord leukoplakia(VCL). Methods 136 VCL patients were examined by white light mode and NBI mode. The pathological characteristics of patients with different properties of lesions were compared, and the value of different types in the diagnosis of VCL properties was analyzed. Results The proportions of smoking, involvement of anterior commissure and involvement range ≥50% in the benign group were lower than those in the malignant group(P <0.05). The NI type of NBI mode had the highest accuracy in diagnosing the properties of VCL lesions, followed by the CHEN type of white light mode. The NI type of NBI mode had the highest Kappa value in identifying VCL properties. Conclusions Both white light mode and NBI mode under electronic laryngoscope have certain value in the identification of VCL properties, and the consistency between NI type of NBI mode and pathological examination is the highest.
