【摘要】 阴茎干性坏疽是临床上一种少见的良性病变，本文报告1例阴茎干性坏疽患者的诊疗过程，并复习相关文献，探讨其病因、临床表现及鉴别诊断、治疗方法。老年男性，69岁，因“阴茎发黑、疼痛1年”就诊。1年前患者发现龟头出现豌豆大小黑色硬结并疼痛不适，院外换药及抗感染治疗效果不佳，坏死区域扩大至整个阴茎并出现排尿疼痛不适。入院后检查发现阴茎干性缺血坏死，行阴茎切除、阴囊部分切除、尿道会阴造口术，术后2周后行蹲位排尿，随访5个月无尿道狭窄发生。阴茎干性坏疽临床较少见，在临床诊疗中，需仔细查找病因并进行针对治疗，在保守治疗无效时应及时手术处理。更多还原

【Abstract】 Penile dry gangrene is a rare benign lesion in clinical practice. This article reports the diagnosis and treatment process of a patient with penile dry gangrene and reviews the relevant literatures to explore its etiology, clinical manifestations, differential diagnosis, and treatment methods. A 69-year-old elderly male presented with "blackening and pain of the penis for 1 year". One year ago, the patient noticed a pea-sized black hard nodule on the glans, accompanied by discomfort and pain. Wound dressing change and anti-infection treatments outside the hospital were ineffective, leading to an expansion of the necrotic area to the entire penis, along with pain and discomfort during urination. Upon admission, examination revealed penile dry ischemic necrosis, and the patient underwent penile amputation, partial scrotal resection, and perineal urethrostomy. The patient was able to urinate in a squatting position 2 weeks after surgery, and no occurrence of urethral stricture were showed at 5 months follow-up. Penile dry gangrene is clinically rare. In clinical diagnosis and treatment, it is essential to carefully identify the underlying causes of the disease and provide targeted treatment. When conservative treatment is ineffective, timely surgical treatment should be performed.更多还原
