【摘要】 目的 评价高危人乳头瘤病毒（HPV）E6/E7mRNA结合宫颈细胞倍体检查在宫颈恶性肿瘤筛查中的意义。方法选择取得2020年3月至2021年3月在某人民医院妇科门诊就诊以及妇科住院部治疗进行宫颈癌筛查的女性患者628例，收集病人的宫颈脱落细胞，将所有得到的细胞标本同时做两种化验，即HPV E6/E7 mRNA+宫颈细胞倍体检查，得到的检查结果中如果有一个提示阳性，那么再进一步在阴道镜指引下完成宫颈活检，活检的病理结果将作为最终的诊断，把得到的每一步检查结果进行逆向分析，探索只使用一项检查和选择两者结合时分别对宫颈肿瘤及早发现产生的不同评价。结果 单一评价方法时:HPV E6/E7 mRNA检测敏感性(96.55%)最高，其特异性为（75.10%）、准确度为(79.10%)，细胞倍体分析的阴性预测值(95.94%)最高。两者结合评价时：其特异性(84.98%)、阳性预测值(55.81%)和准确度（84.57%）最高。结论 HPV E6/E7 mRNA和宫颈细胞倍体相结合共同评价时，看其在特异性、阳性预测值和准确性三方面的表现，呈现出的最后效果好于单一评价方法，建议将两者联合筛查应用于宫颈恶性肿瘤临床筛查中。更多还原

【Abstract】 Objective To analyze the application value of human papillomavirus(HPV) E6/E7mRNA combined with cervical cell DNA ploidy analysis in cervical cancer screening.Methods A total of 628 female patients who received cervical cancer screening at the gynecology outpatient department and gynecology inpatient department of a certain people’s hospital from March 2020 to March2021 were selected, and their cervical exfoliated cells were collected. All the obtained cell specimens were simultaneously subjected to two tests, namely HPV E6/E7 mRNA and cervical cell ploidy examination. If one of the test results indicated positive, then a cervical biopsy was further completed under the guidance of colposcopy. The pathological result of the biopsy was regarded as the final diagnosis. The results of each step of the examination were analyzed in reverse to explore the different evaluations of early detection of cervical tumors when using only one test and when combining the two.Results Among the single detection methods, the HPV E6/E7 mRNA was the highest(96.55%), the specificity was 75.10%, the accuracy was 79.10%, and the negative predictive value of cell ploidy DNA analysis was the highest(95.94%). The specificity(84.98%), positive predictive value(55.81%) and accuracy(84.57%) of the two combined detection schemes were the highest.Conclusion The combined detection of HPV E6/E7 mRNA and cervical cell DNA ploidy analysis has better effect than the single method in terms of specificity, positive predictive value and accuracy. It is suggested that the combined screening of the two types of HPV E6/E7 mRNA and cervical cell DNA ploidy analysis can be used in the clinical screening of cervical malignant tumors.更多还原
