微小RNA-452-5p及SRY盒相关基因7在食管癌组织中的表达水平及与临床病理参数和预后的关系研究

【摘要】 目的 探讨微小RNA-452-5p(miR-452-5p)及SRY盒相关基因7(SOX7)在食管癌组织中的表达水平以及临床病理参数和预后的关系。方法 选取2017年6月至2019年5月期间我院收治的106例食管癌患者作为研究对象。检测食管癌患者miR-452-5p、SOX7 mRNA的表达,分析癌组织中miR-452-5p、SOX7 mRNA表达与临床病理参数的关系,采用Kaplan-Meier法进行生存分析,并采用COX单因素和多因素分析影响食管癌预后的危险因素。结果 相比于癌旁组织,癌组织中的miR-452-5p表达水平更高(P<0.05),SOX7 mRNA表达水平更低(P<0.05)。相比于肿瘤Ⅰ~Ⅱ期、无淋巴结转移、浸润深度(Tis~T2)食管癌患者,miR-452-5p在有淋巴结转移、肿瘤分期Ⅲ期、浸润深度(T3~T4)癌组织中有更高的表达水平,SOX7 mRNA表达水平更低(P<0.05)。Kaplan-Meier生存曲线分析显示,患者3年总生存率miR-452-5p高表达组为30.4%(17/56),miR-452-5p低表达组为54.0%(27/50),累积总生存率差异有统计学意义(Log-rankχ~2=6.082,P=0.014);SOX7 mRNA高表达组和SOX7 mRNA低表达组患者患者3年总生存率分别为53.7%(29/54)、28.8%(15/52),累积总生存率差异有统计学意义(Log-rank χ~2=6.742,P=0.009)。COX单因素回归分析结果显示,miR-452-5p、SOX7 mRNA、肿瘤分期、浸润深度、淋巴结转移与食管癌预后有关。COX多因素回归分析结果显示,影响食管癌预后的独立因素有淋巴结转移、SOX7 mRNA低表达、肿瘤分期Ⅲ期、miR-452-5p高表达、浸润深度(T3~T4)。结论 miR-452-5p在食管癌组织中呈高表达、SOX7 mRNA在食管癌组织中呈低表达,其水平与淋巴结转移、肿瘤分期、浸润深度有关,且密切关联食管癌患者预后。

【Abstract】 Objective To investigate the expression levels of microRNA-452-5p(miR-452-5p) and SRY box related gene 7(SOX7) in esophageal cancer tissue and their relationship with clinical pathological parameters and prognosis. Methods 106 esophageal cancer patients admitted to our hospital from June 2017 to May 2019 were selected as the study subjects. The expression of miR-452-5p and SOX7 mRNA are detected in esophageal cancer patients, the relationship between miR-452-5p and SOX7 mRNA expression in cancer tissue and clinical pathological parameters are analyzed. Survival analysis was conducted using the Kaplan Meier method, and COX univariate and multivariate analyses were used to identify risk factors affecting the prognosis of esophageal cancer. Results Compared to adjacent tissues, the expression level of miR-452-5p is higher in cancer tissues(P<0.05), and the expression level of SOX7 mRNA is lower(P<0.05). Compared to patients with stage Ⅰ-Ⅱ tumors, no lymph node metastasis, and deep invasion(Tis-T2) esophageal cancer, miR-452-5p has higher expression levels in cancer tissues with lymph node metastasis, stage Ⅲ tumors, and deep invasion(T3-T4), and lower expression levels of SOX7 mRNA(P<0.05). Kaplan Meier survival curve analysis showed that the overall 3-year survival rate of patients in the miR-452-5p high expression group was 30.4%(17/56), while in the miR-452-5p low expression group it was 54.0%(27/50). The difference in cumulative overall survival rate was statistically significant(Log-rank χ~2=6.082, P=0.014); The 3-year overall survival rates of patients in the SOX7 mRNA high expression group and the SOX7 mRNA low expression group were 53.7%(29/54) and 28.8%(15/52), respectively. The difference in cumulative overall survival rates was statistically significant(Log-rank χ~2=6.742, P=0.009). The results of COX univariate regression analysis showed that miR-452-5p, SOX7 mRNA, tumor stage, depth of infiltration, lymph node metastasis are related to the prognosis of esophageal cancer. The results of COX multivariate regression analysis showed that independent factors affecting the prognosis of esophageal cancer include lymph node metastasis, low expression of SOX7 mRNA, stage Ⅲ tumor stage, high expression of miR-452-5p, and depth of infiltration(T3~T4). Conclusion MiR-452-5p is highly expressed in esophageal cancer tissue, while SOX7 mRNA is low in esophageal cancer tissue. Its level is related to lymph node metastasis, tumor staging, and depth of infiltration, and is closely related to the prognosis of esophageal cancer patients.
