超声引导下腔内射频消融术联合硬化剂注射治疗下肢静脉曲张的临床效果
Clinical Effect of Ultrasound-Guided Intracavitary Radiofrequency Ablation Combined with Sclerosing Agent Injection in Treatment of Lower Limb Varicose Veins
【摘要】 目的:分析超声引导下腔内射频消融术联合硬化剂注射治疗下肢静脉曲张的临床效果。方法:选取2022年10月—2023年10月银川市第一人民医院收治的下肢静脉曲张患者110例作为研究对象,采用随机数字表法分为对照组和研究组,各55例。对照组采用大隐静脉高位结扎剥脱术治疗,研究组采用超声引导下腔内射频消融术联合硬化剂注射治疗。比较两组治疗效果。结果:术后12 h、24 h、3 d,两组疼痛评分下降,且研究组低于对照组(P<0.05)。术后12 h,两组缓激肽、P物质水平下降,且研究组低于对照组(P<0.05)。术后12 h,两组心率、平均动脉压(MAP)均高于术前,但研究组心率、MAP低于对照组(P<0.05);对照组血氧饱和度(SpO2)降低,研究组SpO2高于对照组(P<0.05)。研究组并发症总发生率低于对照组(P<0.05)。结论:超声引导下腔内射频消融术联合硬化剂注射治疗下肢静脉曲张的临床效果良好,可减轻疼痛应激反应,减少并发症,并且对血流动力学影响小。
【Abstract】 Objective: To analyze the clinical effect of ultrasound-guided intracavitary radiofrequency ablation combined with sclerosing agent injection in the treatment of lower limb varicose veins. Methods: A total of 110 patients with lower limb varicose veins admitted to the First People’s Hospital of Yinchuan from October 2022 to October 2023 were selected as the study subjects.They were divided into control group and study group according to random number table method, with 55 cases in each group. The control group was treated with high ligation and stripping of great saphenous vein, and the study group was treated with ultrasound-guided intracavitary radiofrequency ablation combined with sclerosing agent injection. The treatment effects were compared between the two groups. Results: At 12 h, 24 h and 3 d after surgery, the pain scores in the two groups decreased, and the pain scores in the study group were lower than those in the control group(P<0.05). At 12 h after surgery, the levels of bradykinin and substance P in the two groups decreased, and the levels in the study group were lower than those in the control group(P<0.05).At 12 h after surgery, the heart rate and mean arterial pressure(MAP) in the two groups were higher than those before surgery, but the heart rate and MAP in the study group were lower than those in the control group(P<0.05); the blood oxygen saturation(SpO2)in the control group decreased, and the SpO2in the study group was higher than that in the control group(P<0.05). The total incidence of complications in the study group was lower than that in the control group(P<0.05). Conclusion: Ultrasound-guided intracavitary radiofrequency ablation combined with sclerosing agent injection has good clinical effect in the treatment of lower limb varicose veins, and can relieve pain stress response, reduce complications, and have little impact on hemodynamics.
