血脂异常对南京市社区2型糖尿病患者血糖控制的影响

【摘要】 目的:了解血脂异常对南京市社区2型糖尿病患者血糖控制的影响。方法:采用多阶段分层整群抽样方法,随机抽取南京市18岁以上社区糖尿病患者5 663例作为研究对象,采用多因素Logistic回归分析血脂异常对糖尿病患者血糖控制的作用。结果:5 663例糖尿病患者空腹血糖控制率为39.8%(95%CI 38.5%~41.1%),其中血脂正常患者控制率为43.5%(95%CI 41.6%~45.4%),血脂异常患者控制率为36.6%(95%CI 34.9%~38.3%)。经调整年龄、性别、城乡、受教育水平、职业、高血压、吸烟、饮酒、体质指数、体力活动、糖尿病家族史和规律服药后,血脂异常患者空腹血糖控制率是血脂正常者的0.70倍(95%CI 0.62~0.79,P<0.001)。进一步按性别、城乡和是否规律使用降糖药分层分析显示,各亚组分析结果与总体情况一致。结论:血脂异常会降低社区2型糖尿病患者的血糖控制达标率。

【Abstract】 Objective: To investigate the association between dyslipidemia and glycemic control among community patients with type 2 diabetes. Methods:Totally 5 663 community patients with type 2 diabetes(T2D) were randomly selected from urban and rural communities by multi-stage stratified cluster random sampling. Multivariable Logistic regression models were employed to estimate the associations between dyslipidemia and glycemic control among T2D patients. Results: The glycemic control rate was 39.8%(95%CI 38.5%-41.1%), while that of T2D patients with normal blood lipid and T2D patients with dyslipidemia were 43.5%(95%CI 41.6%-45.4%), 36.6%(95%CI 34.9%-38.3%), respectively. After adjustment for potential confounders such as age, gender, residence etc, T2D patients with dyslipidemia were at 0.70(95%CI 0.62-0.79, P<0.001) times odds to have glycaemia under control than patients with normal blood lipid. Furthermore, when stratified separately by gender, residence and use of anti-diabetes agents, Similar outcomes were observed from all sub-groups. Conclusion: Dyslipidemia might have a negative effect on glycemic control among T2D patients in Nanjing China.
