高危儿母婴巨细胞病毒的监测与预后研究
Surveillance and prognosis of cytomegalovirus in high-risk infants and infants
【摘要】 目的 研究高危儿母婴巨细胞病毒的监测与预后情况。方法 选取2019年8月至2023年12月我院出生并住院的100例小儿,根据人巨细胞病毒(HCMV)抗体检测结果将其分为双阳组30例[HCMV IgM抗体(HCMV-IgM)、IgG抗体(HCMV-IgG)均阳性],单阳组39例(HCMV-IgM阴性、HCMV-IgG阳性),双阴组31例(HCMV-IgM、HCMV-IgG均阴性)。比较3组小儿的发育商(DQ)与智力指数(MI)评分、耳声发射(OAE)听力与自动听性脑干反应(AABR)耳朵通过率、身高、体重、尿液HCMV-DNA阳性率、母乳汁HCMV-DNA阳性率、母亲血清HCMV-IgG阳性率。结果 3组3月龄、6月龄小儿的DQ、MI评分比较,差异均有统计学意义(均P<0.05)。双阴组3月龄、6月龄小儿的DQ、MI评分均高于单阳组、双阳组,差异均有统计学意义(均P<0.05)。单阳组3月龄、6月龄小儿的DQ、MI评分均高于双阳组,差异均有统计学意义(均P<0.05)。3组小儿OAE、AABR耳朵通过率比较,差异均无统计学意义(均P>0.05)。3组3月龄、6月龄小儿身高、体重比较,差异均无统计学意义(均P>0.05)。3组小儿尿HCMV-DNA阳性率、母乳汁HCMV-DNA阳性率、母亲血清HCMV-IgG阳性率比较,差异均有统计学意义(均P<0.05)。结论 HCMV感染能直接影响小儿生长发育水平。HCMV抗体检测可为高危儿HCMV管理提供理论依据,有利于提升高危儿临床保健技术。
【Abstract】 Objective To study the monitoring and prognosis of cytomegalovirus(CMV) in high-risk infants and their mothers. Methods A total of 100 children born and hospitalized in our hospital from August 2019 to December 2023 were selected and divided into the double positive group with 30 cases [both HCMV IgM antibody(HCMV-IgM) and IgG antibody(HCMV-IgG) were positive], single negative group with 39 cases(HCMV-IgM negative, HCMV-IgG positive) and double negative group with 31 cases(HCMV-IgM negative, HCMV-IgG negative). The scores of developmental quotient(DQ) and mental index(MI), ear passage rate of otoacoustic emissions(OAE) and automated auditory brainstem response(AABR), height, weight, positive rate of urine HCMV-DNA, positive rate of HCMV-DNA in maternal milk, positive rate of HCMV-IgG in maternal serum were compared in 3 groups. Results There were significant differences on DQ and MI scores of children aged 3 months and 6 months in 3 groups(all P<0.05). The DQ and MI scores of children aged 3 months and 6 months in double negative group were higher than those in single positive group and double positive group, and the differences were statistically significant(all P<0.05). The DQ and MI scores of children aged 3 months and 6 months in the single positive group were higher than those in the double positive group, and the differences were statistically significant(all P<0.05).There was no significant difference in ear passing rate of OAE and AABR among 3 groups(all P>0.05).There was no significant difference in the height and weight of children aged 3 months and 6 months in 3 groups(P>0.05).The positive rate of urine HCMV-DNA,HCMV-DNA in maternal milk, HCMV-IgG in maternal serum in 3 groups were statistically significant(all P<0.05). Conclusion HCMV infection can directly affect the growth and development level of children.HCMV antibody detection can provide theoretical basis for HCMV management of high-risk infants and is conducive to improving clinical health care technology of high-risk infants.
