通腑合剂对腑实热结型重症急性胰腺炎患者肠黏膜屏障功能及炎症因子的影响
Effects of Tongfu mixture on intestinal mucosal barrier function and inflammatory factor in sever acute pancreatitis of heat stagnation and Fu-organ excess type
【摘要】 目的:探讨通腑合剂对重症急性胰腺炎患者的肠屏障功能及相关炎症因子的影响。方法:将52例重症急性胰腺炎患者随机分为对照组(n=26)和研究组(n=26)。两组疗程均为10 d,比较胃肠功能恢复情况、并发症发生率及急性生理与慢性健康评分(APACHEⅡ)评分,检测肠黏膜屏障功能指标、炎症因子、胰酶指标。结果:治疗后,研究组胃肠功能恢复时间及APACHEⅡ评分优于对照组(均P<0.05)。与治疗前比较,两组治疗后血浆内毒素、二胺氧化酶(DAO)、D-乳酸、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-8(IL-8)、血淀粉酶、血脂肪酶及尿淀粉酶显著降低(均P<0.05),且研究组明显低于对照组(均P<0.05)。研究组总并发症发生率低于对照组(P<0.05)。结论:通腑合剂应用于重症急性胰腺炎疗效显著,能明显改善临床症状,其机制可能与促进胃肠动力,改善肠黏膜屏障功能,减轻炎症反应,抑制胰酶分泌有关。
【Abstract】 Objective:To study the effect of Tongfu mixture on intestinal barrier function and inflammatory factor of sever acute pancreatitis(SAP).Methods:52 cases of SAP were divided into the control group(n=26) and the study group(n=26).The treatment course of both groups was 10 days.The recovery time of intestine function, incidence of complications, APACHE Ⅱ score, intestinal mucosal barrier function indexes, inflammatory factors and trypsin were compared between the two groups.Results:After treatment, the gastrointestinal function recovery time and APACHE Ⅱ score in the study group were better than those in the control group(all P<0.05).Compared with before treatment, plasma endotoxin, diamine oxidase(DAO),D-lactate, CRP,TNF-α,IL-6,IL-8,blood amylase, blood lipase and urine amylase were significantly decreased in both groups(all P<0.05),and the study group was significantly lower than the control group.The complication rate of the study group was lower than that of the control group(all P<0.05).Conclusion:Tongfu mixture has a good curative effect in the treatment of sever acute pancreatitis, which can significantly improve the clinical symptoms.Its mechanism may be related to improving the gastrointestinal motility, promoting the recovery of intestinal mucosal barrier function, reducing inflammatory reaction, and inhibiting pancreatic enzyme secretion.
