血清25-(OH)D3、sVCAM-1、MMP-9与慢性乙型肝炎病毒感染孕妇合并妊娠期肝内胆汁淤积症的关系研究

【摘要】 目的:探讨血清25-羟维生素D3[25-(OH)D3]、可溶性血管细胞黏附分子-1(sVCAM-1)、基质金属蛋白酶-9(MMP-9)与慢性乙型肝炎(CHB)病毒感染孕妇合并妊娠期肝内胆汁淤积症(ICP)的关系。方法:选择2021年1月至2023年12月华中科技大学同济医学院附属湖北妇幼保健院收治的的120例CHB病毒感染合并ICP孕妇作为研究对象(合并组),以未合并ICP的100例CHB病毒感染孕妇作为对照(未合并组)。比较两组血清25-(OH)D3、sVCAM-1、MMP-9水平。单因素和多因素Logistic回归分析发生ICP的相关因素。结果:合并组孕妇血清sVCAM-1、MMP-9均高于未合并组(P<0.05),25-(OH)D3低于未合并组(P<0.05)。多因素Logistic回归分析结果显示,血清sVCAM-1和MMP-9升高是CHB病毒感染孕妇合并ICP的危险因素,25-(OH)D3升高是CHB病毒感染孕妇合并ICP的保护因素(P均<0.05)。结论:CHB病毒感染孕妇血清25-(OH)D3、sVCAM-1、MMP-9与ICP的发生密切相关。

【Abstract】 Objective: To investigate the relationship between serum 25-hydroxyvitamin D3 [25-(OH)D3], soluble vascular cell adhesion molecule-1(sVCAM-1), matrix metalloproteinase-9(MMP-9) and intrahepatic cholestasis of pregnancy(ICP) in pregnant women with chronic hepatitis B(CHB) virus infection. Methods: 120 pregnant women with CHB virus infection with ICP admitted to the Maternal and Child Health Hospital of Hubei Province, Tongji Medical College, Huazhong University of Science and Technology from January 2021 to December 2023 were selected as the study subjects(combined group), and 100 pregnant women with CHB virus infection without ICP were selected as control(non-combined group), the levels of serum 25-(OH)D3, sVCAM-1 and MMP-9 were compared between two groups. The related factors of ICP were analyzed by univariate and multivariate Logistic regression analysis.Results: The levels of serum sVCAM-1 and MMP-9 in combined group were higher than those in non-combined group(P<0.05), and 25-(OH)D3 was lower than that in non-combined group(P<0.05). Multivariate Logistic regression analysis showed that, elevated serum sVCAM-1 and MMP-9 were risk factors for ICP in pregnant women with CHB virus infection, and elevated 25-(OH)D3 was a protective factor for ICP in pregnant women with CHB virus infection(all P<0.05). Conclusion: Serum 25-(OH)D3, sVCAM-1 and MMP-9 in pregnant women with CHB virus infection are closely relate to the occurrence of ICP.
