重复经颅磁刺激联合手-臂双侧强化训练对偏瘫脑瘫儿童上肢功能及日常活动能力的影响

【摘要】 目的 探讨对偏瘫脑瘫儿童规范展开重复经颅磁刺激(repetitive transcranial magnetic stimulation, rTMS)联合手-臂双侧强化训练对促进上肢功能恢复、提高日常活动能力的影响。方法 选取2022年12月—2023年12月安徽医科大学附属第一医院收治的80例偏瘫脑瘫患儿,按不同训练方案分为两组,每组40例,对照组进行常规康复训练,观察组在对照组的基础上进行rTMS联合手-臂双侧强化训练。对比两组上肢协调能力、上肢关节活动能力、日常生活活动能力、生活质量。结果 训练后,观察组抓握评分为(44.27±6.63)分、视觉-运动整合能力评分为(106.39±19.78)分,均高于对照组的(38.34±7.78)分、(96.36±21.14)分,差异有统计学意义(t=3.669、2.191,P均<0.05)。训练后,观察组上肢技巧质量量表评分、日常生活活动能力评分、中文版儿童生活质量量表评分均高于对照组,差异有统计学意义(P均<0.05)。结论 rTMS联合手-臂双侧强化训练可显著改善脑瘫偏瘫患儿的上肢精细功能及关节活动度,提升双手协调能力及日常生活能力、生活质量。

【Abstract】 Objective To explore the effect of repetitive transcranial magnetic stimulation(rTMS) combined with hand-arm bilateral intensive training on promoting the recovery of upper limb function and improving the ability of daily activities in children with hemiplegic cerebral palsy. Methods A total of 80 children with hemiplegic cerebral palsy admitted to the First Affiliated Hospital of Anhui Medical University from December 2022 to December 2023 were selected and divided into two groups according to different training schemes, with 40 cases in each group. The control group received routine rehabilitation training, and the observation group received rTMS combined with hand-arm bilateral intensive training on the basis of the control group. The upper limb coordination ability, upper limb joint activity ability, daily living activity ability and quality of life were compared between the two groups. Results After training, the score of grasping was(44.27±6.63) points and the score of visual-motor integration ability was(106.39±19.78) points in the observation group, which were higher than(38.34±7.78) points and(96.36±21.14) points in the control group, the differences were statistically significant(t=3.669, 2.191, both P<0.05). After training, the scores of Quality of Upper Extremity Skills Test, Modified Barthel Index and Chinese Version Pediatric Quality of Life Inventory in the observation group were higher than those in the control group, and the differences were statistically significant(all P<0.05). Conclusionr TMS combined with hand-arm bilateral intensive training can significantly improve the upper limb fine function and joint mobility of children with hemiplegic cerebral palsy, and improve the coordination ability of both hands, daily living ability and quality of life.
