彩色多普勒超声在血液透析动静脉内瘘血管狭窄诊断中的应用
Application of Color Doppler Ultrasound in the Diagnosis of Arteriovenous Fistula Complications on Hemodialysis
【摘要】 目的 探究彩色多普勒超声在血液透析动静脉内瘘并发症诊断中的应用价值。方法 选取2020年6月—2022年6月在百色市人民医院应用自体动静脉内瘘进行血液透析的80例患者为研究对象,使用彩色多普勒超声对患者动静脉内瘘进行检查,以血管造影为金标准,分析彩色多普勒超声在人工动静脉内瘘并发症中的诊断效能。结果 血管造影结果显示,术后有12例患者发生狭窄,彩色多普勒超声诊断发生狭窄的患者有10例。彩色多普勒超声诊断血液透析患者动静脉内瘘术后并发症的灵敏度、特异度、阳性预测值、阴性预测值、准确度分别为83.33%(10/12)、98.53%(67/68)、90.91%(10/11)、97.10%(67/69)、96.25%(77/80),kappa值为0.848,与血管造影具有较高的一致性。结论彩色多普勒超声能对动静脉内瘘血管形态学的改变进行动态监测,对术后并发症的诊断具有较高的诊断价值。
【Abstract】 Objective To explore the application value of color Doppler ultrasound in the diagnosis of complications of arteriovenous fistula in hemodialysis. Methods From June 2020 to June 2022, a total of 80 hemodialysis patients with autologous arteriovenous fistula in Baise People’s Hospital were selected as the research objects. Color Doppler ultrasound was used to examine the arteriovenous fistula of the patients. With angiography as the gold standard, the diagnostic efficacy of color Doppler ultrasound in the complications of artificial arteriovenous fistula was analyzed. Results The results of angiography showed that 12 patients had stenosis after operation, and 10 patients had stenosis diagnosed by color Doppler ultrasound. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of color Doppler ultrasound in the diagnosis of postoperative complications of arteriovenous fistula in hemodialysis patients were 83.33%(10/12), 98.53%(67/68),90.91%(10/11), 97.10%(67/69) and 96.25%(77/80), respectively. The kappa value was 0.848, which had a high consistency with angiography. Conclusion Color Doppler ultrasound can dynamically monitor the morphological changes of arteriovenous fistula, and has high diagnostic value for the diagnosis of postoperative complications.
