2017—2023年北京市朝阳区病毒性胃肠炎聚集性疫情流行特征分析

【摘要】 目的 分析2017—2023年北京市朝阳区病毒性胃肠炎聚集性疫情的流行病学特征和变化趋势,探讨影响其疫情规模的相关危险因素,为完善其防控措施提供科学依据。方法 收集2017—2023年北京市朝阳区报告的且确定为由胃肠炎病毒引起的聚集性疫情资料,采用描述性流行病学方法比较分析其流行趋势、季节性变化、高发场所等;应用logistic回归对影响疫情发生规模的相关因素进行单因素和多因素分析,探讨可能的危险因素。结果 2017—2023年朝阳区共接报并处置病毒性胃肠炎聚集性疫情585起,累计报告病例5 509例,无死亡病例。疫情整体呈现先降后升、短期波动趋势,表现为隔年低发趋势;每年疫情高峰集中于3—5月和9—11月,有明显的季节性。发生疫情较多的前5位街道/地区分别为望京街道、来广营地区、劲松街道、东坝地区和豆各庄地区;疫情发生场所主要为托幼机构和学校,主要累及5岁以下儿童(占47.57%);导致病毒性胃肠炎聚集性疫情的病原体主要为诺如病毒(480起,占82.22%)和札如病毒(78起,占13.33%);影响托幼机构和学校聚集性疫情规模的危险因素有病例有公共场所活动史(OR=1.890,95%CI=1.125~3.176)、发生疫情后消毒不规范(OR=4.437,95%CI=2.732~7.206)。结论 2017—2023年北京市朝阳区报告的病毒性胃肠炎聚集性疫情以诺如病毒感染导致为主。建议在每年开学季督促朝阳区内的托幼机构和小学内的疫情防控措施落实,从而有效控制疫情的发生发展。

【Abstract】 Objective To analyze the epidemiological characteristics and trends of viral gastroenteritis outbreaks in Chaoyang district, Beijing, from 2017 to 2023, and to explore the risk factors affecting the scale of viral gastroenteritis aggregation outbreaks, so as to provide scientific basis for improved prevention and control measures.Methods Information on aggregated outbreaks reported and characterized as caused by gastroenteritis viruses in Chaoyang district, Beijing, from 2017 to 2023 was collected, and descriptive epidemiological method were used to comparatively analyze their epidemiological trends, seasonal changes, and places of high incidence, etc. Logistic regression was applied to conduct single-factor and multifactorial analyses of relevant factors affecting the scale of outbreaks and to explore the possible risk factors.Results A total of 585 viral gastroenteritis outbreaks were reported and disposed of in the region from 2017 to 2023, with a cumulative total of 5 509 reported cases and no fatal cases. The overall trend of outbreaks displayed a pattern of initial decline followed by an increase with short-term fluctuations, showing an alternating-year low incidence trend; the peak of outbreaks each year was concentrated in March-May and September-November, with obvious seasonality. The top 5 streets and townships with more outbreaks were Wangjing subdistrict, Laiguangying region, Jinsong subdistrict, Dongba region and Dougezhuang region; outbreaks occurred mainly in childcare institutions and schools, affecting mostly children under 5 years old(47.57%); pathogens leading to clustering of viral gastroenteritis were mainly norovirus(480 cases, 82.22%) and zarotovirus(78 cases, 13.33%); risk factors affecting the scale of aggregated outbreaks in childcare institutions and schools included cases with a history of activity in public places(OR=1.890, 95% CI=1.125~3.176), and irregular disinfection after the occurrence of outbreaks(OR=4.437, 95% CI=2.732~7.206).Conclusions The viral gastroenteritis outbreaks reported in Chaoyang district, Beijing, from 2017 to 2023 were mainly caused by norovirus infection. It is recommended to supervise the epidemic prevention and control education for childcare institutions and elementary school to implement various gastroenteritis epidemic prevention and control measures in Chaoyang district during the school season every year, so as to effectively control the development of viral gastroenteritis epidemics.
