【摘要】 目的 探讨卒中后抑郁患者血浆中血脂参数总胆固醇、低密度脂蛋白水平变化，及其对卒中后抑郁的预测价值。方法 检索中国知网、维普数据库、万方数据库等中文数据库以及Cochrane Library、Web of Science、EMbase和PubMed英文数据库获取血脂参数与卒中后抑郁相关的前瞻性研究，检索时间截止至2023年12月20日。使用R 4.3.2软件“meta R Package”“robvis R Package”进行统计学分析。结果 最终总胆固醇与卒中后抑郁相关文献筛选出11篇，低密度脂蛋白与卒中后抑郁相关文献筛选出12篇，共计2 168例卒中非抑郁，743例卒中后抑郁患者。研究结果发现卒中后抑郁患者总胆固醇TC(SMD=0.14,95%CI 0.04～0.24)和低密度脂蛋白LDL(SMD=0.19,95%CI 0.10～0.28)显著高于卒中非抑郁患者。研究异质性小，无明显发表偏倚，结果具有稳定性。结论 卒中后抑郁的发病可能与总胆固醇和低密度脂蛋白的升高有关，在临床工作中可对合并以上血脂生化指标的的卒中患者进行卒中后抑郁的早期干预，从而改善患者的预后和生活质量。更多还原

【Abstract】 Objective To Explore the changes in plasma lipid parameters such as total cholesterol and low-density lipoprotein levels in patients with post-stroke depression, and their predictive value for post-stroke depression.Methods The CNKI,VIP database, Wanfang database, Cochrane Library, Web of Science, Embase, and PubMed English were searched to obtain prospective studies on the relationship between blood lipid parameters and post-stroke depression.The retrieval time was up to December 20,2023.The R 4.3.2 software “meta R package” and “robvis R package” were used for statistical analysis.Results Finally, eleven articles related to total cholesterol and post-stroke depression and twelve articles related to low-density lipoprotein and post-stroke depression were screened, a total of two thousand one hundred and sixty-eight cases of non-depression after stroke and seven hundred and forty-three cases of post-stroke depression patients.The results showed that total cholesterol(SMD=0.14,95%CI 0.04～0.24) and low-density lipoprotein(SMD=0.19,95%CI 0.10～0.28) in patients with post-stroke depression were significantly higher than those in patients without post-stroke depression.The heterogeneity of the study was small, there was no obvious publication bias, and the results were stable.Conclusion The incidence of post-stroke depression may be related to the increase of total cholesterol and low-density lipoprotein.In clinical work, early intervention of post-stroke depression can be carried out in stroke patients with the above blood lipid biochemical indicators, so as to improve the prognosis and quality of life of patients.更多还原
