丙种球蛋白对新型冠状病毒肺炎患者中性粒细胞/淋巴细胞比值、PCT、CRP的影响
【摘要】 目的 探讨丙种球蛋白对新型冠状病毒肺炎(COVID-19)患者中性粒细胞/淋巴细胞比值(NLR)、PCT、CRP的影响。方法选取2022年12月至2023年2月COVID-19患者80例,随机分为对照组和观察组,各40例。两组患者均采用抗病毒治疗方案、早期抗凝、糖皮质激素抗炎及氧疗等对症治疗。观察组在对照组基础上加静脉注射人用丙种球蛋白针,治疗5 d。动态检测两组治疗期间动血常规、PCT、CRP等感染指标变化情况,动态检测动脉血气、肺部影像评估感染控制情况和不良反应发生情况。结果 与治疗前比较,两组患者NLR、PCT、CRP数值均下降,差异有统计学意义(P<0.05);治疗后两组比较,观察组NLR值下降更明显,差异有统计学意义(P<0.05)。两组患者住院时间、不良事件发生率,差异均无统计学意义(P>0.05)。结论 丙种球蛋白能降低新型冠状病毒肺炎患者的NLR,有助于病情改善。
【Abstract】 Objective To investigate the effect of gamma globulin on neutrophil/lymphocyte ratio(NLR), procalcitonin(PCT), C-reactive protein(CRP) in patients with novel coronavirus(COVID-19) pneumonia. Methods Select 80 hospitalized patients with COVID-19 infection in our hospital from December 2022 to February 2023, and randomly divide them into a control group and an observation group, with 40 patients in each group.Both groups of patients received symptomatic treatment including antiviral therapy, early anticoagulation, corticosteroid anti-inflammatory therapy, and oxygen therapy according to the standard treatment plan. The observation group received intravenous injection of human immunoglobulin needles in addition to the control group, and was treated synchronously for 5 days. Dynamically monitor the changes in blood routine, PCT, CRP, and other infection indicators during the treatment period of two groups, and dynamically monitor arterial blood gas and lung imaging to evaluate infection control and adverse reaction occurrence. Results Compared with before treatment, the values of NLR, PCT, and CRP in both groups of patients decreased significantly(P<0.05). After treatment, the observation group showed a more significant decrease in NLR values compared to the control group, with a statistically significant difference(P<0.05). There was no statistically significant difference in the decrease of PCT and CRP values. Conclusion Gamma globulin can reduce the NLR of patients with COVID-19 pneumonia, which is helpful to improve the patient’s condition.
