远端尺桡关节融合结合尺骨远端假关节成形术治疗远端尺桡关节炎
Modified Sauvé-Kapandji surgery for treatment of the distal radioulnar arthritis
【摘要】 目的 探讨关节镜下远端尺桡关节融合结合尺骨远端假关节成形术(改良Sauvé-Kapandji术)治疗远端尺桡关节炎的临床疗效。方法 采用关节镜下改良Sauvé-Kapandji术治疗12例远端尺桡关节炎患者。记录远端尺桡关节融合情况、并发症发生情况、腕关节疼痛VAS评分及患侧握力,测量关节活动度,采用Mayo评分评价腕关节功能。结果 患者均获得随访,时间8~36个月。远端尺桡关节均融合,时间11~28(18.0±6.3)周。无截骨断端再骨化、骨桥形成等情况发生。术后6个月,疼痛VAS评分较术前降低(P<0.05),腕关节活动度和前臂旋转度较术前改善(P<0.05),患侧握力较术前改善(P<0.05)。术后6个月腕关节功能Mayo评分为55~95(76.3±14.7)分,其中优6例,良3例,中2例,差1例,优良率9/12。结论 采用关节镜下改良Sauvé-Kapandji术治疗远端尺桡关节炎融合率高,疼痛缓解明显,关节功能恢复较好。
【Abstract】 Objective To investigate the clinical effect of distal radioulnar joint fusion combined with ulnar distal pseudoarthroplasty(modified Sauvé-Kapandji surgery) for treatment of distal radioulnar arthritis under arthroscopy.Methods The 12 patients with distal radioulnar arthritis were treated by modified Sauvé-Kapandji surgery under arthroscopy.Distal radioulnar joint fusion, complications, pain VAS of the wrist joint and the grip strength of affected side were recorded, joint motion was measured, and wrist function was evaluated by Mayo score.Results All patients were followed up for 8~36 months.Distal radioulnar joint fusion lasted in 11~28(18.0±6.3) weeks.There was no re-ossification of osteotomy broken end, bone bridge formation.Six months after surgery, the pain VAS was decreased than before surgery(P<0.05),the wrist joint motion and forearm rotation range were improved, compared with those before surgery(P<0.05),and the grip strength of affected side was improved than the preoperation(P<0.05).At 6 months after surgery, the Mayo score of wrist joint function was 55~95(76.3±14.7) points, of which 6 cases were excellent, 3 good, 2 medium, and 1 poor, with an excellent-good rate of 9/12.Conclusions In the treatment of distal radioulnar arthritis, the modified Sauvé-Kapandji surgery under arthroscopy has high fusion rate, obvious pain relief and good joint function recovery.
