RUNX3 mRNA、IFN-γ、IgE水平检测在毛细支气管炎患儿中的临床意义

【摘要】 目的 分析runt相关转录因子3(RUNX3)mRNA、干扰素-γ(IFN-γ)、免疫球蛋白E(IgE)水平检测在毛细支气管炎患儿中的临床意义。方法 选取2019年3月至2022年1月在该院诊治的104例毛细支气管炎患儿作为观察组(轻度28例、中度52例、重度24例),另选取同期体检健康的57例婴幼儿作为对照组。采用荧光定量PCR检测RUNX3 mRNA水平,采用酶联免疫吸附试验(ELISA)检测血清中IFN-γ、IgE水平。比较两组RUNX3 mRNA、IFN-γ、IgE水平,绘制受试者工作特征(ROC)曲线评估RUNX3 mRNA、IFN-γ、IgE水平对小儿毛细支气管炎的诊断价值,比较不同病情程度患儿RUNX3 mRNA、IFN-γ、IgE水平,采用Spearman相关分析RUNX3 mRNA、IFN-γ、IgE水平与病情程度的相关性。结果 与对照组相比,观察组RUNX3 mRNA、IFN-γ水平明显较低,IgE水平明显较高,差异均有统计学意义(P<0.001)。ROC曲线分析结果显示,RUNX3 mRNA、IFN-γ、IgE诊断小儿毛细支气管炎的曲线下面积(AUC)分别为0.731(95%CI:0.655~0.797)、0.850(95%CI:0.780~0.901)、0.867(95%CI:0.805~0.915)。不同程度毛细支气管炎患儿RUNX3 mRNA、IFN-γ水平表现为轻度组>中度组>重度组,IgE水平表现为轻度组<中度组<重度组,任意两组间比较,差异均有统计学意义(P<0.05)。Spearman相关分析结果显示,RUNX3 mRNA、IFN-γ水平与病情程度呈负相关(r=-0.618、-0.547,P<0.001),IgE水平与病情程度呈正相关(r=0.574,P<0.001)。结论 RUNX3 mRNA、IFN-γ、IgE水平对小儿毛细支气管炎有较好的诊断价值,且与病情程度有相关性,对毛细支气管炎的早期诊断、病情进展有一定的评估作用。

【Abstract】 Objective To analyze the clinical significance of runt-related transcription factor 3(RUNX3) mRNA,interferon-γ(IFN-γ) and immunoglobulin E(IgE) levels detection in children patients with bronchiolitis.Methods A total of 104 children patients with bronchiolitis diagnosed and treated in this hospital from March 2019 to January 2022 were selected as the observation group(28 cases of mild bronchiolitis, 52 cases of moderate bronchiolitis and 24 cases of severe bronchiolitis),and 57 healthy infants and young children undergoing physical examination during the same period were selected as the control group.The RUNX3 mRNA level was detected by fluorescence quantitative PCR.The serum IFN-γ and IgE levels were detected by enzyme-linked immunosorbent assay(ELISA).The levels of RUNX3 mRNA and IFN-γ were compared between the two groups.The receiver operating characteristic(ROC) curve was drawn to evaluate the diagnostic values of RUNX3 mRNA,IFN-γ and IgE levels for children bronchiolitis.The RUNX3 mRNA,IFN-γ and IgE levels were compared among different disease conditions of children patients.Spearman correlation was used to analyze the correlation between RUNX3 mRNA,IFN-γ and IgE level with the disease course degree.Results Compared with the control group, the levels of RUNX3 mRNA and IFN-γ in the observation group were significantly lower, the IgE level was significantly higher, and the differences were statistically significant(P<0.001).The ROC curve analysis results showed that the area under the curve(AUC) of RUNX3 mRNA,IFN-γ and IgE for diagnosing bronchiolitis was 0.731(95%CI:0.655-0.797),0.850(95%CI:0.780-0.901) and 0.867(95%CI:0.805-0.915) respectively.The RUNX3 mRNA and IFN-γ levels were manifested by the mild group > moderate group>severe group.The IgE level was manifested by the mild group <moderate group <severe group, and the difference between any two groups was statistically significant(P<0.05).Spearman correlation analysis results showed that RUNX3 mRNA and IFN-γ levels were negatively correlated with the condiction degree of the disease(r=-0.618,-0.547,P<0.001),while the IgE level was positively correlated with the condiction degree severity(r=0.574,P<0.001).Conclusion The RUNX3 mRNA,IFN-γ and IgE have the good diagnostic value for bronchiolitis in children, moreover which are correlated with the disease severity degree and have a certain evaluating role for the early diagnosis and progress of bronchiolitis.
