2018—2022年北京市丰台区人源弯曲菌流行状况及毒力基因与耐药分析

【摘要】 目的 了解北京市丰台区2018—2022年弯曲菌的流行特征、ST型分布、毒力基因及耐药基因携带情况。方法 采用双孔滤膜法对食源性腹泻病例粪便标本进行弯曲菌的分离培养,采用实时荧光PCR、全自动生化及飞行质谱进行鉴定,采用琼脂稀释法进行药敏试验。全基因组数据通过数据库pubMLST、VFDB、CARD比对,识别ST型别、毒力基因及耐药基因。结果 丰台区2018—2022年腹泻病例弯曲菌感染率为8.15%(125株)。弯曲菌感染呈6—7月、9—10月双高峰,空肠弯曲菌(空弯)101株检出50种ST型,其中20种属新型别,结肠弯曲菌(结弯)24株检出15种ST型,其中5种属新型别。空弯、结弯分别检出124种、84种毒力相关基因,分别检出26、17种耐药基因。空弯与结弯gyrA:T86I、tet(O)携带率(>85%)均较高。空弯和结弯对TET、NAL、CIP耐药率均较高(>65.00%);空弯对ERY、AZI、STR、TEL、CLI较为敏感(耐药率均<2.00%),结弯对CHL敏感(耐药率为0)。结论 丰台区弯曲菌感染处于中等水平。空弯ST型呈遗传多样化特征,且有较强的致病潜力;未来应持续监测弯曲菌分型、致病性和耐药性,指导临床合理用药。

【Abstract】 Objective To investigate the epidemiological characteristic, ST typing, virulence gene and drug-resistant genes in Campylobacter isolates from Fengtai District, Beijing Municipality from 2018 to 2022. Methods Campylobacter was isolated from foodborne diarrheal patients′ stool samples using the diplopore and filter method, cultured and characterized using real-time fluorescence PCR assay, fully automated biochemical analysis and flight mass spectrometry.Antimicrobial susceptibility tests were performed using the agar dilution method and the whole-genome sequencing data were aligned with the pubMLST,VFDB and CARD databases to identify the ST types, virulence gene and drug-resistant genes. Results The prevalence of Campylobacter infection was 8.15%(125 isolates) among diarrheal patients in Fengtai District from 2018 to 2022,and the prevalence peaked between June and July and between September and October.A total of 50 ST types were identified in 101 C.jejuni isolates, including 20 new types, and 15 ST types were identified in 24 C.coli isolates, including 5 new types.A total of 124 virulence genes were detected in C.jejuni,including 26 drug-resistant genes, and 84 virulence genes were detected in C.coli,including 17 drug-resistant genes.The prevalence of gyrA:T86I and tet(O) genotypes was high in C.jejuni and C.coli,and the prevalence of resistance to TET,NAL and CIP was all high in C.jejuni and C.coli(all > 65.00%).In addition, C.jejuni was susceptible to ERY,AZI,STR,TEL and CLI(< 2.00% prevalence of drug resistance),and C.coli was susceptible to CHL(0 prevalence of drug resistance). Conclusions The prevalence of Campylobacter infection is moderate in Fengtai District.The ST types of Campylobacter appear a genetic diversity, which a high pathogenic potential.Persistent monitoring of Campylobacter types, pathogenicity and drug resistance is required to guide the clinical rational drug uses.
