护士建言行为的潜在剖面分析及影响因素研究

【摘要】 目的 分析护士建言行为潜在类别及影响因素。方法 采用建言行为量表、变革型领导量表、组织承诺量表对山东省8所三级医院2 300名在职护士进行调查,采用潜在剖面分析建立了护士建言行为潜在类别模型,采用Logistic回归分析探讨护士建言行为潜在类别的影响因素。结果 护士的建言行为水平可以分为“高抑制性-消极建言组”(22.65%)、“高促进性-中等水平建言组”(61.30%)、“高促进性-积极建言组”(16.04%)3种潜在类别。多元Logistic回归分析显示,不同工龄、职称、聘用形式、变革型领导和组织承诺水平的护士建言行为存在显著差异(P<0.01或P<0.05)。结论 护士的建言行为处于中等偏上水平,可分为3个潜在类别。影响护士建言行为潜在类别的因素较多,护理管理者可针对不同建言行为类别的影响因素,对护士展开针对性干预,以提高建言行为水平。

【Abstract】 Objective To analyze the potential categories and influencing factors of nurses’ advocacy behaviors.Methods A survey of 2,300 working nurses in eight tertiary hospitals in Shandong Province was conducted using the Advocacy Behavior Scale, the Transformational Leadership Scale, and the Organizational Commitment Scale.Three models of potential categories of nurses’ advocacy behaviors were established using latent profile analysis, and logistic regression analyses were used to explore the factors influencing the potential categories of nurses’ advocacy behaviors.Results Nurses’ level of advocacy behaviors could be classified into three potential categories, including highly inhibitire-negative constructive group(22.65%),highly facilitative-moderate level constructive group(61.30%),and highly facilitative-positive constructive group(16.04%).Multivariate Logistic regression analyses revealed significant differences in the advocacy behavior of nurses with different length of service, job title, form of employment, transformational leadership and level of organizational commitment(P<0.01 or P<0.05).Conclusion Nurses’ advocacy behaviors were at a moderately high level and could be classified into 3 potential categories.There are many factors affecting the potential categories of their advocacy behavior, and nursing managers could target interventions to improve nurses’ advocacy behavior by addressing the related influencing factors.
