前列腺调强放疗分段计划中采用二段合成设计与独立设计第二程计划的剂量学比较
Dosimetry Comparison between a Two-Stage Synthetic Design and an Independently Designed Second-Course Plan in a Staged Prostate Modulated Radiotherapy Program
【摘要】 目的:比较二段合成设计与独立设计第二程计划在前列腺癌患者化疗治疗中的应用效果,探讨其计量学差异。方法:选取阳江市人民医院2021年7月1日至2022年8月30日收治的30例前列腺癌患者作为研究对象,所有患者均进行分段的调强放疗计划,将二段合成设计作为对照组,独立设计方案作为观察组,比较不同治疗方案下两组患者放射治疗的机器跳数、子野数目、靶区和危及器官的剂量学差异作为观察指标。结果:对照组与观察组患者子野数目无明显差异,观察组机器跳数少于对照组,观察组患者靶区和危及器官的剂量学低于对照组患者,P<0.05,有统计学差异。结论:相较于传统二段合成设计放疗而言,独立设计方案应用于前列腺癌放射治疗疗效更加显著,所用剂量较少,同时所需的机器跳数更少,值得在临床推广实施。
【Abstract】 Objective:To compare the application effect of two-stage synthetic design and independently designed second course plan in chemotherapy treatment of prostate cancer patients, and to discuss the metrological differences. Methods:Thirty patients with prostate cancer admitted to Yangjiang People’s Hospital from July 1, 2021 to August 30, 2022 were selected as the study objects. All patients underwent a segmented intensity modulated radiotherapy program, with a two-stage composite design as the control group and an independent design as the observation group. The number of machine hops, number of subfields, dosimetry of target area and organs at risk were compared between the two groups under different treatment regiments. Results:There was no significant difference in the number of subfields between the control group and the observation group, the number of machine hops in the observation group was less than that in the control group, and the dosimetry of the target area and the organs at risk in the observation group was lower than that in the control group, P<0.05, with statistical difference. Conclusion:Compared with traditional two-stage synthetic design radiotherapy, the independent design scheme is more effective in prostate cancer radiotherapy, with fewer doses and fewer machine hops required, and is worthy of clinical promotion and implementation.
