早产儿a EEG监测的临床意义及其影响因素

【摘要】 目的 探讨早产儿振幅整合脑电图(aEEG)监测的临床意义,并分析影响aEEG异常的相关因素。方法 前瞻性选择2021年1月至2023年12月云浮市妇幼保健院收治的300例早产儿作为研究对象,a EEG异常早产儿作为异常组,其余早产儿作为正常组,比较不同胎龄早产儿a EEG异常发生率,比较异常组和正常组早产儿的临床资料,并采用多因素Logistic回归分析影响早产儿a EEG异常的相关因素。结果 300例早产儿共检出195例a EEG异常,总异常率为65.00%;胎龄28~31周的早产儿a EEG总异常率为90.00%,明显高于胎龄32~33周的70.27%,以及胎龄34~36周的53.61%,且胎龄32~33周早产儿aEEG总异常率明显高于胎龄34~36周,差异均有统计学意义(P<0.05);异常组、正常组早产儿的出生体质量、妊娠期高血压疾病、BE、喂养方式、新生儿呼吸窘迫综合征、新生儿窒息比较差异均有统计学意义(P<0.05);多因素Logistic回归分析结果显示,出生体质量、妊娠期高血压疾病、碱剩余(BE)、喂养方式、新生儿呼吸窘迫综合征、新生儿窒息均是导致早产儿a EEG异常的影响因素(P<0.05)。结论 早产儿a EEG异常率较高,且影响因素较多,包括出生体质量、妊娠期高血压疾病、BE、喂养方式、新生儿呼吸窘迫综合征、新生儿窒息,应受到临床关注。

【Abstract】 Objective To investigate the clinical significance of amplitude-integrated electroencephalography(aEEG) monitoring in preterm infants, and analyze the factors affecting aEEG abnormalities. Methods A total of300 preterm infants admitted to Yunfu Maternal and Child Health Hospital from January 2021 to December 2023 were prospectively selected as research subjects, which were divided into the abnormal group(preterm infants with abnormal a EEG) and the normal group(preterm infants with normal aEEG). The incidence of aEEG abnormalities in preterm infants of different gestational ages were compared, and the clinical data of preterm infants were compared between the abnormal group and the normal group. Multivariate logistic regression was used to analyze the factors affecting a EEG abnormalities in preterm infants. Results Among the 300 preterm infants, 195 cases of a EEG abnormalities were detected, with the total abnormal rate of 65.00%. The total abnormal rate of a EEG in preterm infants at 28 to 31 weeks of gestational age was 90.00%, which was significantly higher than 70.27% in preterm infants at 32 to 33 weeks of gestational age and 53.61% at 34 to 36 weeks of gestational age; the total abnormal rate at 32 to 33 weeks of gestational age was significantly higher than that at 34 to 36 weeks of gestational age; the differences were statistically significant(P<0.05).There were statistically significant differences in birth weight, hypertensive disease during pregnancy, BE, feeding pattern, neonatal respiratory distress syndrome, and neonatal asphyxia between the abnormal group and the normal group(P<0.05). Multivariate logistic regression analysis showed that birth weight, hypertensive disease during pregnancy, base excess(BE), feeding methods, neonatal respiratory distress syndrome, and neonatal asphyxia were influencing factors for abnormal a EEG in preterm infants(P<0.05). Conclusion Premature infants have a higher incidence of a EEG abnormalities, which are influenced by multiple factors, including birth weight, hypertensive diseases during pregnancy, BE,feeding pattern, neonatal respiratory distress syndrome, and neonatal asphyxia.
