社区老年人群睡眠障碍与肌肉减少症的相关性研究
The Correlation between Sleep Disorders and Sarcopenia in a Community-based Elderly Population
【摘要】 目的 从睡眠时间和睡眠质量两个方面研究睡眠障碍与肌肉减少症的关系。方法 对2023年3月—2023年8月于重庆市主城区及区县共计5个社区卫生中心体检的老年人群进行调查,以匹兹堡睡眠质量指数(PSQI)量表评估睡眠质量,以睡眠时长分为睡眠时间较短(<5 h)、睡眠时间正常(5~<7 h)和睡眠时间较长(≥7 h)三类,肌肉减少症的诊断采用亚洲肌肉减少症工作小组推荐的诊断标准。结果 共计纳入622例受试者,其中80例(12.9%)患肌肉减少症。不同睡眠时长的受试者肌肉减少症患病率不同(P<0.05),睡眠时间较长的受试者肌肉减少症患病率高于睡眠时间正常的受试者(P<0.05);睡眠质量好和睡眠质量差的受试者肌肉减少症患病率比较,差异无统计学意义(P>0.05)。logistic回归方程结果显示:未调整协变量时,睡眠时间较长的老年女性肌肉减少症患病率是睡眠时间正常者的2.486倍,在调整年龄、体质量指数、婚姻史、工作状况、文化程度、高血压、糖尿病后,睡眠时间较长的老年女性肌肉减少症患病率是睡眠时间正常者的2.818倍;而在老年男性受试者中,睡眠时间与肌肉减少症之间无相关性(P>0.05)。结论 长睡眠时间的老年人相较于正常睡眠时间者更易患肌肉减少症,此结果在女性中较为显著。
【Abstract】 Objective Exploring the relationship between sleep disorders and sarcopenia in terms of both sleep duration and sleep quality.Methods A survey was conducted on the elderly population who came to the main city and districts of Chongqing Municipality from March 2023 to August 2023 for medical checkups in a total of five community health centers.Sleep quality was assessed by the Pittsburgh sleep quality index(PSQI) scale.Sleep duration was categorized as short sleep(<5 h),normal sleep(5-<7 h) and long sleep(≥7 h).The diagnosis of sarcopenia was based on the diagnostic criteria recommended by the Asian Sarcopenia Working Group.Results A total of 622 subjects were included in this study, including 80(12.9%) suffering from sarcopenia.The prevalence of sarcopenia differed among subjects with different sleep duration(P<0.05).The prevalence of sarcopenia was higher in subjects with longer sleep duration than in subjects with normal sleep duration(P<0.05).Comparison of the prevalence of sarcopenia between subjects with good and poor sleep quality showed no statistically significant difference(P>0.05).Logistic regression equations showed that the prevalence of sarcopenia in older women who slept longer than those with a normal sleep duration was 2.486 times higher when not adjusted for covariates.The prevalence of sarcopenia in older women who slept longer was 2.818 times that of those with normal sleep duration, after adjusting for age, body mass index, marital history, work status, education, hypertension, and diabetes mellitus.In contrast, there was no correlation between sleep duration and sarcopenia in older male subjects(P>0.05).Conclusion Elderly people who sleep longer are more likely to suffer from sarcopenia than those who sleep a normal amount of time, and this finding is more pronounced in women.
