艾拉莫德联合硫酸羟氯喹对原发性干燥综合征患者红细胞沉降率、辅助性T细胞17/调节性T细胞免疫失衡的影响
Effects of iguratimod combined with hydroxychloroquine sulfate on erythrocyte sedimentation rate and immune imbalance of helper T cell 17/regulatory T cell in patients with primary Sjogren’s syndrome
【摘要】 目的 探讨艾拉莫德联合硫酸羟氯喹对原发性干燥综合征(primary Sjogren’s syndrome,pSS)患者红细胞沉降率(erythrocyte sedimentation rate,ESR)、辅助性T细胞(helper T cell,Th)17/调节性T细胞(regulatory T cell,Treg)免疫失衡的影响。方法 选择2019年1月至2023年12月于我院接受治疗的pSS患者199例,采用随机数表法分为试验组(n=100,硫酸羟氯喹+艾拉莫德)和对照组(n=99,硫酸羟氯喹)。比较两组治疗前后临床疗效、ESR、Th17、Treg、唾液流率、泪液流率、泪膜破碎时间、EULAR SS患者报告指数(EULAR Sjogren’s syndrome patient reported index,ESSPRI)、EULAR SS疾病活动指数(EULAR Sjogren’s syndrome disease activityindex,ESSDAI)评分及不良反应发生情况。结果 治疗后,试验组总有效率高于对照组(91.00% vs 79.80%,P<0.05)。两组ESR、Th17水平较前显著降低(P<0.05),且试验组低于对照组(P<0.05);两组Treg水平较前明显升高(P<0.05),且试验组高于对照组。两组唾液流率、泪液流率、泪膜破碎时间较前明显升高(P<0.05),且试验组高于对照组(P<0.05)。两组ESSPRI、ESSDAI评分较前明显降低(P<0.05),且试验组低于对照组(P<0.05)。治疗期间,试验组和对照组不良反应发生率无显著差异(3.00% vs 5.05%,P>0.05)。结论 艾拉莫德联合硫酸羟氯喹对pSS疗效较佳,可有效改善ESR、Th17/Treg免疫失衡。
【Abstract】 Objective To investigate the effect of iguratimod combined with hydroxychloroquine sulfate on erythrocyte sedimentation rate(ESR) and immune imbalance of helper T cell(Th)17/regulatory T cell(Treg) in patients with primary Sjogren’s syndrome(pSS).Methods A total of 199 pSS patients treated in our hospital from January 2019 to December 2023 were selected and divided into experimental group(n=100,hydroxychloroquine sulfate+iguratimod) and control group(n=99,hydroxychloroquine sulfate) by random number table method.Clinical efficacy,ESR,Th17,Treg,salivary flow rate,tear flow rate,tear film breaking time,and EULAR Sjogren’s syndrome patient reported index(ESSPRI),EULAR Sjogren’s syndrome disease activity index(ESSDAI) and the incidence of adverse reactions were compared between the two groups before and after treatment.Results After treatment,the total effective rate of experimental group was higher than that of control group(91.00% vs 79.80%,P<0.05).The levels of ESR and Th17 in two groups were significantly lower than before(P<0.05),and those in experimental group were lower than those in control group(P<0.05).The level of Treg in both groups were significantly higher than before(P<0.05),and the experimental group was higher than the control group.The salivary flow rate,tear flow rate and tear film breaking time of the two groups were significantly increased(P<0.05),and the experimental group was higher than the control group(P<0.05).The scores of ESSPRI and ESSDAI in two groups were significantly lower than before(P<0.05),and the experimental group was lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the experimental group and the control group during treatment(3.00% vs 5.05%,P>0.05).Conclusion Iguratimod combined with hydroxychloroquine sulfate has better effect on pSS,and can effectively improve the immune imbalance of ESR and Th17/Treg.
