【摘要】 目的 探讨事件相关电位P3b成分在帕金森病(PD)认知障碍诊断的应用价值。方法 比较PD组和健康对照组P3b指标的差异，评估P3b指标与PD组的临床资料及认知障碍的依存关系。结果 帕金森病痴呆(PDD)组的P3b潜伏期较帕金森病轻度认知障碍(PD-MCI)组延长(P<0.05),较健康对照组和帕金森病认知正常(PD-CN)组明显延长(P<0.01)。PD组的P3b潜伏期与蒙特利尔认知评估(MoCA)评分呈负相关(r=-0.506,P<0.01),视空间与执行力、延迟回忆对P3b潜伏期产生负向影响(t=-4.775,P<0.01;t=-2.186,P<0.05)。PD组的P3b潜伏期与年龄、Hoehn-Yahr分级、UPDRS-Ⅲ评分呈正相关(r=0.191,P<0.05;r=0.262,P<0.01;r=0.218,P<0.05),与受教育年限呈负相关(r=-0.340,P<0.01);PD组的P3b潜伏期与性别、帕金森病病程无明显的相关关系(r=0.041,P>0.05;r=0.084,P>0.05)。结论 认知损害越严重，P3b潜伏期越长，P3b潜伏期可以作为PD认知障碍的量化指标，对于早期识别PD认知障碍有意义。更多还原

【Abstract】 Objective To explore the application value of P3b component of event-related potentials in the diagnosis of cognitive impairment of Parkinson’s disease(PD) patients.Methods The difference in P3b indicators between PD group and healthy control group was compared, and the dependence relationship of P3b indicator on clinical data and cognitive impairment in the PD group was evaluated.Results The P3b latency of Parkinson’s disease dementia(PDD)group was longer than that of PD mild cognitive impairment(PD-MCI) group(P<0.05), and significantly longer than that of the healthy control group and PD normal cognition(PD-CN) group(P<0.01).The P3b latency of the PD group was negatively correlated with the Montreal cognitive assessment(MoCA) score(r=-0.506, P<0.05), visuospatial and executive functions and delayed recall had negative impact on the P3b latency(t=-4.775, P<0.01; t=-2.186, P<0.05).The P3b latency in the PD group was positively correlated with age, Hoehn Yahr grading, and UPDRS-Ⅲ score(r=0.191,P<0.05;r=0.262,P<0.01;r=0.218,P<0.05), and negatively correlated with years of education(r=-0.340, P<0.01); The P3b latency in the PD group was not significantly correlated with gender and the course of Parkinson’s disease(r=0.041,P>0.05;r=0.084,P>0.05).Conclusion The more severe the cognitive impairment, the longer the P3b latency.P3b latency can serve as a quantitative indicator of PD cognitive impairment, which is meaningful for early identification of PD cognitive impairment.更多还原
