代谢相关脂肪性肝病危险患病影响因素分析及患病风险预测模型的构建

【摘要】 目的 分析代谢相关脂肪性肝病(MAFLD)的影响因素,并构建MAFLD患病风险预测模型。方法 以完成腹部超声检查的2 600例健康体检者为研究对象,根据超声检查的结果将研究对象分为MAFLD组和对照组。采用logistic回归分析筛选MAFLD患病影响因素,构建列线图模型。结果 多因素logistic回归分析结果显示,男性、体质量指数≥24 kg/m~2、高水平血清尿酸和甘油三酯、高血压病、糖尿病病史为MAFLD的独立危险因素。基于上述因素构建了MAFLD患病风险预测列线图,曲线下面积及校准曲线显示该模型具有良好诊断性能,临床决策分析显示该模型具有良好的临床净收益。结论 本研究构建的MAFLD风险预测模型具有较高的准确度,可为MAFLD的早期筛查和干预提供参考依据。

【Abstract】 Objective To identify the influencing factors of metabolic associated fatty liver disease(MAFLD) and develop a risk prediction model. Methods The study included 2 600 healthy individuals who underwent abdominal ultrasound examination. Based on the results of the ultrasound, participants were divided into the MAFLD group and a control group. Logistic regression analysis was used to identify factors influencing the prevalence of MAFLD, and a nomogram model was constructed. Results The multivariable logistic regression analysis results indicated that male, body mass index ≥ 24 kg/m~2, elevated serum uric acid and triglyceride levels, as well as a history of hypertension and diabetes, were identified as independent risk factors for MAFLD. Based on these risk factors, a nomogram risk prediction model for MAFLD was constructed. The area under the curve and calibration curves demonstrated that the model had good diagnostic performance, and clinical decision analysis showed that the model had good clinical net benefit. Conclusion The MAFLD risk prediction model developed in this study has high accuracy and can serve as a reference for early screening and intervention of MAFLD.
