磁共振细胞外容积分数联合PDFF成像在NAFLD食蟹猴模型中对早期肝脏纤维化的诊断价值

【摘要】 目的 探讨肝脏磁共振细胞外容积分数(extracellular volume,ECV)及质子密度脂肪分数(proton density fat fraction,PDFF)在非酒精性脂肪性肝病早期肝纤维化(F1期)食蟹猴中的诊断价值?方法 纳入经肝穿病理证实的早期肝纤维化食蟹猴104只(F1期组)和10只健康对照组,对比两组肝ECV和PDFF值差异及对早期肝纤维的诊断效能?结果 F1期组ECV?PDFF值均高于对照组(P<0.001)?ECV和PDFF与肝NAS评分呈中度相关(r ECV-小叶炎症评分=0.432 P<0.001,r PDFF-脂肪变性评分=0.628 P<0.001),ECV在早期肝纤维?PDFF在脂肪定量评估中均具有良好的诊断效能(AUC-ECV=0.884,P<0.001,AUC-PDFF=0.991,P<0.001)?结论 MRI-ECV联合PDFF成像在NAFLD食蟹猴中对早期肝纤维化及脂肪定量评估具有巨大潜力?

【Abstract】 Objective To investigate the diagnostic value of magnetic resonance extra cellular volume fraction(ECV) and proton density fat fraction(PDFF) in cynomolgus monkey model of nonalcoholic fatty liver disease(NAFLD).Methods 104 cynomolgus monkeys(F1 group) with ea rly liver fibrosis(stage F1) were confirmed by liver puncture pathology and 10 normal cynomolgus monkeys(normal group)were included for abdominal T1 mapping and PDFF imaging.The liver magnetic resonance extracellular volume fraction(MRI-ECV)and the fat fra ction of the two groups were compared.Results the MRI-ECV and PDFF value in the F1 group were higher than those in the normal group,and there was significa nt difference between the two groups(PECV<0.001,PPDFF<0.001).There was a moderate correlation between MRI-ECV and MRI-PDFF with NAS scores of liver(rECV-lobular inflammation=0.432 P<0.001,rPDFF-steatosis=0.628 P<0.001),demonstrating good diagnostic efficacy in liver fibrosis and fat assessment.Conclusion MRI-ECV combined with MRI-PDFF has great potential for quantitative evaluation of early liver fibrosis and fat fraction in NAFLD cynomolgus mon key model.
