基于Mini-CEX评价的教学方法在中医硕士专业学位研究生住院医师培训中的应用

【摘要】 目的 探讨迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)在中医硕士专业学位研究生住院医师培训中的应用效果。方法 选取2022年1月1日至2023年8月31日于北京中医药大学第二临床医学院中医内科进行轮转的中医硕士专业学位研究生来源的住院医师83人为研究对象。随机分为观察组(n=39)和对照组(n=48),观察组采取Mini-CEX+常规教学,对照组采取常规教学。结果 观察组在病史采集、体格检查、沟通技能、西医诊治、中医四诊、辨证论治、人文关怀和整体表现各项评分随着住培时间的增长而提高。观察组在体格检查、技能操作和诊治能力3个项目得分高于对照组;在病历书写项目得分低于对照组。观察组教学满意度多个维度高于对照组,以上差异具有统计学意义(P<0.05)。Mini-CEX评价得到观察组住院医师广泛认可(100%)。结论 Mini-CEX评价对于夯实住院医师的临床基本功,提高其诊疗能力有确切的效果。

【Abstract】 Objective To explore the effect of Mini-CEX on the clinical rotation of resident physicians in Chinese Medicine. Methods A total of 83 resident doctors who were rotated from January 1, 2022 to August 31,2023 in the Second Clinical Medical College of Beijing University of Traditional Chinese Medicine were selected.The observation group(n=39) and the control group(n=48) were randomized. The observation group received MiniCEX and routine instruction. The control group took routine teaching. Results With the increase of the duration of the training, the scores of observation group increased significantly in the medical history acquisition, physical examination, communication skills, western medicine diagnosis and treatment, the four diagnostic methods, syndrome differentiation and treatment, humanistic care and overall performance. The scores of physical examination, skill operation and ability of diagnosis and treatment in the observation group were higher than those in the control group. The score of medical history writing was lower than that of the control group. The teaching satisfaction of the observation group was higher than that of the control group in many dimensions, and the differences were significant(all P< 0.05). The MiniCEX evaluation was widely accepted by participants(100%). Conclusion The Mini-CEX evaluation has a definite effect on consolidating the clinical basic skills of resident physicians and improving their diagnosis and treatment ability.
