肺磨玻璃结节CT定性及定量指标与肺腺癌浸润程度的关系
Relationship between qualitative and quantitative CT indicators of pulmonary ground glass nodule and the degree of invasion of lung adenocarcinoma
【摘要】 目的 分析肺磨玻璃结节CT定性及定量指标与肺腺癌浸润程度的关系。方法 将187例肺磨玻璃结节患者根据术后病理结果分为浸润前组39例、微浸润组54例、浸润组94例。所有患者术前行CT影像学检查,比较3组患者CT定性及定量指标,绘制受试者工作特征(ROC)曲线,分析CT定量指标对肺腺癌浸润程度的预测价值。结果 3组患者CT定性指标中病灶形态、边缘分叶特征、血管特征及瘤-肺界面比较,差异均有统计学意义(P﹤0.01)。3组患者CT定量指标中磨玻璃结节最大径、实性成分最大径及平均CT值比较,差异均有统计学意义(P﹤0.01)。磨玻璃结节最大径、实性成分最大径及平均CT值预测肺腺癌浸润程度的曲线下面积(AUC)分别为0.808、0.952、0.738。结论 CT定性及定量指标与肺腺癌浸润程度有关。
【Abstract】 Objective To analyze the relationship between qualitative and quantitative CT indicators of pulmonary ground glass nodule and the degree of invasion of lung adenocarcinoma. Method A total of 187 patients with pulmonary ground glass nodules were divided into pre-invasion group(39 cases), micro-invasion group(54 cases) and invasion group(94 cases) according to postoperative pathological results. All patients underwent preoperative CT imaging examinations, the qualitative and quantitative CT indicators of the three groups were compared, and receiver operating characteristic(ROC) curve was drawn to analyze the predictive value of quantitative CT indicators for the degree of invasion of lung adenocarcinoma. Result There were significant differences in lesion morphology, marginal lobed features, vascular features and tumor-lung interface among the three groups of qualitative CT indicators(P<0.01). There were statistically significant differences in the maximum diameter of ground glass nodule, the maximum diameter of solid component and the average CT value among the three groups of quantitative CT indicators(P<0.01). The area under the curve(AUC) of the maximum diameter of ground glass nodule, the maximum diameter of solid component and the average CT value in predicting the degree of invasion of lung adenocarcinoma were 0.808, 0.952 and 0.738, respectively. Conclusion Qualitative and quantitative CT indicators are related to the degree of invasion of lung adenocarcinoma.
