【摘要】 目的：观察电针联合重复经颅磁刺激治疗卒中后丘脑痛的效果。方法：选取广东省第二中医院2023年1月～2024年1月收治的卒中后丘脑痛患者60例，按照随机数字表法分为对照组和观察组各30例。对照组给予常规西药治疗，观察组在对照组基础上给予电针+重复经颅磁刺激治疗，连续治疗6周。比较两组临床总有效率、痛感、生活质量、心理状态、平衡能力、日常生活能力、躯干控制能力、大脑中动脉平均血流速度、血清中疼痛相关因子水平变化。结果：观察组总有效率高达93.33%，高于对照组的73.33(P<0.05)；治疗后同一时间点与对照组比较，观察组VAS评分均显著降低，SF-36评分显著升高，HAMD评分显著降低，BBS评分、BI指数、TCT评分显著升高，VA、BA显著升高（P<0.05），血清中SP、CGRP水平显著降低（P<0.05）。结论：电针联合重复经颅磁刺激治疗卒中后丘脑痛能显著减轻痛感、抑郁情绪，提高生活质量，控制病情，可能通过增强脑血流、改善外周SP、CGRP的水平发挥镇痛、神经保护作用。更多还原

【Abstract】 Objective:To observe the effect of electroacupuncture combined with repeated transcranial magnetic stimulation on thalamic pain after stroke.Methods:Sixty patients with thalamic pain after stroke admitted to Guangdong Second Traditional Chinese Medicine Hospital from January2023 to January 2024 were selected and divided into control group and observation group with 30 patients in each group according to random number table method.The control group was given conventional western medicine treatment,and the observation group was given acupuncture+repeated transcranial magnetic stimulation+conventional western medicine treatment for 6 weeks.The clinical total response rate,pain,quality of life,mental state,balance ability,daily living ability,trunk control ability,mean middle cerebral artery blood flow velocity,serum pain-related factors were compared between the two groups.Results:The total effective rate of observation group was 93.33%,which was higher than that of control group 73.33%(P<0.05).Compared with the control group at the same time after treatment,VAS scores in the observation group were significantly decreased,SF-36 scores were significantly increased,HAMD score was significantly decreased,BBS score,BI index and TCT score were significantly increased,VA and BA were significantly increased(P<0.05),serum SP and CGRP levels were significantly decreased(P<0.05).Conclusion:The electroacupuncture combined with repeated transcranial magnetic stimulation is effective in the treatment of thalamic pain after stroke,which can significantly reduce pain and depression,improve quality of life,and control the disease.It may play analgesic and neuroprotective effects by enhancing cerebral blood flow and improving the expression of peripheral SP and CGRP.更多还原
