软骨肉瘤的18F-FDG PET/CT征象

【摘要】 目的 总结软骨肉瘤的18F-FDG PET/CT征象,提高对本病的认识。资料与方法 回顾性收集北京大学第三医院2012年10月—2021年12月病理证实的21例软骨肉瘤患者的PET/CT资料。分析病变部位、数目、密度、骨质破坏、周围侵犯等形态学及18F-FDG代谢特点,测量病变最大标准化摄取值(SUVmax)和径线。结果 21例软骨肉瘤中,单发12例,多发9例。初诊14例,其中单发9例,均见软组织肿块伴钙化影,溶骨性破坏7例;多发5例,均见钙化影。复发7例,软组织肿块影6例,钙化影5例。病变18F-FDG代谢不同程度增高,大部分代谢不均匀,体积较大者尤著,呈边缘和(或)间隔代谢增高。8例高-中分化(普通型7例,透明细胞型1例)与8例低分化(间叶性5例,普通型、黏液样、去分化各1例)软骨肉瘤中位SUVmax分别为3.52(3.09,5.33)、9.02(6.52,16.86),差异有统计学意义(Z=3.151,P=0.001)。结论 软骨肉瘤常见溶骨性破坏及钙化,18F-FDG代谢不同程度增高,与病理分级相关。

【Abstract】 Purpose To investigate the 18F-FDG PET/CT features of chondrosarcoma and to increase awareness of the disease. Materials and Methods Retrospective PET/CT data collection was performed on 21 patients at Peking University Third Hospital who had pathologically confirmed chondrosarcoma from October 2012 to December 2021. The morphological characteristics including its location,number, density, destruction of bone, adjacent invasion, and 18F-FDG metabolism were analyzed. The diameter and maximum standardized uptake value(SUVmax) were measured, respectively. Results Nine cases had more than one lesion, while twelve cases had just one. Nine out of the 14 primary cases had a single lesion, these all had a soft tissue mass with calcification, and seven of the cases exhibited osteolytic destruction. Five cases had multiple lesions with calcification. Six of the seven recurrent cases had a soft tissue mass, and five of them exhibited calcification. There were heterogeneous increases in the uptake of 18F-FDG in most cases, presenting as marginal and/or septal uptake,particularly in bigger lesions. The median SUVmax of eight well-moderate lesions(7 conventional, 1 clear cell) and eight poorly differentiated lesions(5 mesenchymal, 1 conventional, 1 myxoid, 1 dedifferentiated) was 3.52(3.09, 5.33) and 9.02(6.52, 16.86), respectively, with statistically significant difference(Z= 3.151, P=0.001). Conclusion Osteolytic destruction and calcification are common in chondrosarcoma,and the 18F-FDG metabolic patterns of tumor may be related to pathological differentiation.
