【摘要】 目的 观察针刺对急性胆囊炎患者腹腔镜切除术后患者恶心呕吐（PONV）、胃肠功能的影响。方法64例急性胆囊炎腹腔镜切除术患者按1∶1比例分对照组与治疗组。对照组术后予常规治疗，治疗组在术后常规治疗基础上联合针刺治疗，比较两组术后5 d PONV发生率、PONV程度，肠鸣音恢复时间、肛门排气时间、排便时间，术后1、5 d切口痛、内脏痛、肩部痛，术前、术后5 d胃动素（MTL）、胃泌素（GS）水平。结果 术后5 d治疗组PONV发生率为6.25%(2例)，低于对照组的25.00%(8例)，PONV程度轻于对照组（P <0.05）；治疗组肠鸣音恢复时间、肛门排气时间、排便时间均短于对照组（P <0.05）。两组术后1 d切口痛、内脏痛、肩部痛差异无统计学意义（P> 0.05），两组术后5 d切口痛、内脏痛、肩部痛均下降，但两组差异无统计学意义（P> 0.05）。术后5 d治疗组MTL、GS水平高于对照组（P <0.05）。结论 针刺治疗急性胆囊炎腹腔镜切除术患者可显著降低PONV的发生率，减轻PONV的程度，促进胃肠功能恢复，改善胃肠激素水平，但缓解疼痛效果不显著。更多还原

【Abstract】 Objective:To observe the effect of acupuncture on postoperative nausea and vomiting(PONV)and gastrointestinal function in patients with acute cholecystitis after cholecystectomy. Methods:64 patients with acute cholecystectomy in our hospital were divided into control group and treatment group according to 1∶1 ratio.The control group received routine treatment after operation,and the treatment group received acupuncture treatment on the basis of routine treatment after operation. The incidence of PONV after 5 d,the PONV degree,recovery time of bowel sounds,time of anal exhaust,defecation time,incision pain after 1,5 d,visceral pain,shoulder pain,and motilin(MTL)and gastrin(GS)levels before and 5 days after operation were compared between the two groups. Results:The incidence of PONV in the treatment group was 2 patients(6.25%),which was lower than 8patients(25.00%)in the control group 5 days after operation,and the degree of PONV was lighter than that in the control group(P < 0.05). The recovery time of bowel sounds,anal exhaust time and defecation time in the treatment group were shorter than those in the control group(P < 0.05). There was no significant difference between the two groups in incision pain,visceral pain and shoulder pain 1 d after operation,and there was no significant difference between the two groups in incision pain,visceral pain and shoulder pain 5 d after operation(P > 0.05). The levels of MTL and GS in the treatment group were higher than those in the control group 5 d after operation(P <0.05). Conclusion:Acupuncture can significantly reduce the incidence and degree of PONV,promote the recovery of gastrointestinal function,and improve the level of gastrointestinal hormones in patients with acute cholecystitis undergoing laparoscopic cholecystectomy,but the pain relief effect is not significant.更多还原
