【摘要】 目的 探讨温阳通络针灸法配合作业疗法对痉挛型脑性瘫痪(简称脑瘫)患儿上肢肌群痉挛状态、肢体运动功能及脑电功率的影响。方法 选取2020年8月至2022年8月我院收治的痉挛型脑瘫患儿113例，随机分为单一组56例和联合组57例。单一组给予作业疗法，联合组给予联合温阳通络针灸法，对比两组临床疗效、上肢肌群痉挛状态、肢体运动功能以及脑电功率指标。结果 联合组总有效率为94.74%(54/57),高于单一组82.14%(46/56),差异有统计学意义(P<0.05)。联合组肱二头肌、喙肱肌、尺侧屈腕肌改良Ashworth肌张力评定量表(MAS)评分均低于单一组，差异有统计学意义(P<0.05)。联合组Carroll上肢功能评定量表(UEFT)、粗大运动功能评估量表(GMFM)、儿童功能独立性评估(WeeFIM)量表评分均高于单一组，差异有统计学意义(P<0.05)。联合组θ波、α/SMR比值功率低于单一组，β波功率高于单一组，差异有统计学意义(P<0.05)。结论 温阳通络针灸法配合作业疗法具有良好的治疗效果，能够有效改善痉挛型脑瘫患儿脑电波，降低肌张力，缓解上肢肌群痉挛状态，促进肢体运动功能恢复。更多还原

【Abstract】 Objective To investigate the effects of Wenyang Tongluo acupuncture and moxibustion combined with occupational therapy on spastic state of upper limb muscle group, limb motor function and EEG power in children with spastic cerebral palsy.Methods A total of 113 children with spastic cerebral palsy admitted to our hospital from August 2020 to August 2022 were chosen and randomly divided into two groups. The 56 cases in the single group were given occupational therapy and 57 cases in the combination group were given combined Wenyang Tongluo acupuncture and moxibustion. The clinical efficacy, spastic state of upper limb muscle group, limb motor function and EEG power indexes of the two groups were compared.Results The total effective rate of combination group(94.74%,54/57) was higher than that of single group(82.14%,46/56)(P<0.05). MAS scores of biceps brachialis, coracobrachialis and flexor carpi ulnar in combination group were lower than those in single group(P<0.05). UEFT, GMFM and WeeFIM scores in combination group were all higher than those in single group(P<0.05). The power of θ wave and α/SMR ratio in combination group was lower than that in single group, and the power of β wave was higher than that in single group(P<0.05).Conclusion Wenyang Tongluo acupuncture and moxibustion combined with occupational therapy has good therapeutic effect, which can effectively improve the brain wave of children with spastic cerebral palsy, reduce muscle tension, relieve spastic state of upper limb muscle group, and promote the recovery of limb motor function.更多还原
