可塑形射频导管在迂曲大隐静脉射频治疗中的应用
Application of Flexible Radiofrequency Catheter in Radiofrequency Therapy of Tortuous Great Saphenous Vein
【摘要】 目的 探讨可塑形射频导管在迂曲大隐静脉射频治疗中的价值。方法 2021年1~12月我科对11例大隐静脉主干迂曲的原发性大隐静脉曲张利用Fcare可塑形射频导管通过迂曲大隐静脉主干进行热消融治疗,功率输出25 W,工作频率4 MHz。观察即刻技术成功率及手术并发症情况。结果 9例顺利完成手术,2例通过主干根部高位结扎的补救方法顺利完成手术。术后1例大腿穿刺点周围疼痛,1例皮下淤血,并发症发生率18.2%(2/11),症状均于1周内缓解,未出现水肿、静脉血栓、感染及神经损伤等严重并发症。术后1、6个月CEAP分级均为C0级1例,C1级8例,C2级2例,与术前(C3级1例,C3级6例,C5级4例)比较有明显改善(Z=-3.127,P=0.002)。术后1、6个月静脉临床严重程度评分(Venous Clinical Severity Score, VCSS)分别为(3.9±0.5)、(1.1±0.5)分,显著低于术前(6.9±1.0)分(均P=0.000)。结论 可塑形射频导管应用于迂曲大隐静脉的射频治疗可行,安全有效。
【Abstract】 Objective To evaluate the value of flexible radiofrequency catheter in the radiofrequency therapy of tortuous great saphenous vein. Methods A total of 11 patients with primary varicose great saphenous vein with tortuous main trunk in our department were retrospectively analyzed. The Fcare plastic radiofrequency catheter was used to perform thermal ablation through the tortuous trunk of the great saphenous vein, with a power output of 25 W and a working frequency of 4 MHz. The technical success rate and surgical complications were observed immediately after operation. Results The surgery was successfully completed in 9 patients, and high ligation of the trunk root was additionally performed to complete the operation in 2 patients. After the operation, 1 patient suffered from pain around the puncture point of the thigh and 1 patient suffered from subcutaneous congestion. The complication rate was 18.2%(2/11). The symptoms were relieved within 1 week. There were no serious complications such as edema, venous thrombosis, infection or nerve injury. The CEAP classification at 1 and 6 months after operation was 1 case of grade C0, 8 cases of grade C1, and 2 cases of grade C2, which was significantly improved as compared to that before operation(1 case of grade C3, 6 cases of grade C4, 4 cases of grade C5; Z=-3.127, P=0.002). The Venous Clinical Severity Score(VCSS) at 1 month and 6 months after operation were(3.9±0.5) and(1.1±0.5) points, respectively, which were significantly lower than those before operation [(6.9±1.0) points, both P=0.000]. Conclusion Use of flexible radiofrequency catheter in radiofrequency therapy of tortuous great saphenous vein is feasible, safe and effective.
