血清hs-CRP、IL-35与胸腰椎骨折患者术后感染发生的相关性
Correlation between serum hs-CRP, IL-35 and postoperative infection in patients with thoracolumbar fractures
【摘要】 目的 检测胸腰椎骨折患者术前血清超敏C-反应蛋白(hs-CRP)、白细胞介素-35(IL-35)水平,分析血清hs-CRP、IL-35与术后感染发生的关系。方法 本研究为前瞻性研究,纳入杭州市萧山区中医院2020年6月—2022年6月治疗的胸腰椎骨折患者260例,入院后,设计基线资料调查问卷,调查患者基线资料,并测定血清hs-CRP、IL-35等实验室指标;术后2周,观察并记录患者术后感染发生情况,根据术后有无感染将患者分为感染组与未感染组,分析血清hs-CRP、IL-35与胸腰椎骨折患者术后感染发生的关系。结果 260例胸腰椎骨折患者均顺利完成本研究,无退出患者;术后2周,260例胸腰椎骨折患者中,发生术后感染18例,占比为6.92%;感染组合并糖尿病占比高于未感染组,手术时间长于未感染组,血清hs-CRP水平高于未感染组,血清IL-35水平低于未感染组,差异有统计学意义(P<0.05);经点二列相关性分析显示,血清hs-CRP水平与胸腰椎骨折患者术后感染成正相关关系(r=0.540,P<0.001),血清IL-35与术后感染成负相关关系(r=-0.621,P<0.001);经logistic回归分析结果显示,血清hs-CRP水平升高是胸腰椎骨折患者术后感染发生的危险因素(OR>1,P<0.05),血清IL-35水平升高是术后感染发生的保护因素(OR<1,P<0.05)。结论 胸腰椎骨折患者术前血清hs-CRP、IL-35与术后感染发生有关。
【Abstract】 Objective This paper aims to detect the serum levels of hypersensitive C-reactive protein(hs-CRP) and interleukin-35(IL-35) in patients with thoracolumbar fracture before operation and analyze the relationship between serum hs-CRP, IL-35 and postoperative infection. Methods This study is a prospective study, including 260 patients with thoracolumbar fracture treated by Xiaoshan District Hospital of Traditional Chinese Medicine from June 2020 to June 2022. After admission, a baseline data questionnaire was designed to investigate the baseline data of patients, and the serum hs-CRP, IL-35 and other laboratory indicators were measured. Two weeks after operation, the incidence of postoperative infection was observed and recorded. The patients were divided into infected group and uninfected group according to the presence or absence of postoperative infection. The relationship between serum hs-CRP, IL-35 and postoperative infection in patients with thoracolumbar fracture was analyzed. Results All 260 patients with thoracolumbar fracture successfully completed the study, and no patient withdrew. Two weeks after operation, 18 of 260 patients with thoracolumbar fracture had postoperative infection, accounting for 6.92%. The proportion of infected group with diabetes was higher than that of uninfected group, the operation time was longer than that of uninfected group, the serum level of hs-CRP was higher than that of uninfected group, and the serum level of IL-35 was lower than that of uninfected group, with the difference statistically significant(P<0.05). The point two column correlation analysis showed that the serum level of hs-CRP was positively correlated with postoperative infection in patients with thoracolumbar fracture(r=0.540, P<0.001), and the serum IL-35 level was negatively correlated with postoperative infection(r=-0.621, P<0.001). The results of logistic regression analysis showed that the increase of serum hs-CRP level was a risk factor for postoperative infection in patients with thoracolumbar fracture(OR>1, P<0.05), and the increase of serum IL-35 level was a protective factor for postoperative infection(OR<1, P<0.05). Conclusion Hs-CRP and IL-35 in patients with thoracolumbar fracture before operation are related to postoperative infection.
