耐碳青霉烯类铜绿假单胞菌感染的临床特征及相关危险因素研究
Clinical characteristics and risk factors of carbapenem-resistant Pseudomonas aeruginosa infection
【摘要】 目的 探讨医院耐碳青霉烯类铜绿假单胞菌(CRPA)感染的临床特征及相关危险因素,为有效防控CRPA感染提供科学依据。方法 收集2020年1月至2022年6月,广东省人民医院分离的CRPA所致感染的患者病例资料60例,采用1∶2病例-对照研究方法,随机收集由碳青霉烯类敏感铜绿假单胞菌(CSPA)所致感染患者资料120例作为对照,分析CRPA感染的临床特征、感染危险因素及患者预后。结果 60株CRPA主要来源于下呼吸道分泌物(30株,50.0%)和中段尿标本(10株,16.7%),集中于重症监护室(26株,43.3%)和心血管外科(13株,21.7%)。经单因素分析,多种因素可能会导致CRPA感染的发生,其中患者应用过氨基糖苷类药物(OR=0.339,95%CI:0.146~0.790,P=0.012)及应用过≥3种抗菌药物(OR=0.247,95%CI:0.068~0.898,P=0.034)可能是导致CRPA感染发生的重要危险因素。CRPA感染的死亡率高于CSPA感染(P=0.003)。结论 CRPA所致感染严重影响患者预后,临床需采取积极措施提升对CRPA的防控能力,同时需加强对各类抗菌药物的管理,做好环境卫生学监测,以减少CRPA感染的发生与院内流行。
【Abstract】 Objective To investigate the clinical characteristics and related risk factors of carbapenem-resistant Pseudomonas aeruginosa(CRPA) infection in hospital, so as to provide scientific basis for effective prevention and control of CRPA.Methods The clinical data of 60 patients with CRPA infection isolated from Guangdong Provincial People’s Hospital were collected from January 2020 to June 2022.By means of case-control study in a 1∶2 ratio, 120 patients with infection caused by carbapenem sensitive Pseudomonas aeruginosa(CSPA) were randomly collected in the same period, and the clinical characteristics, risk factors, prognosis of CRPA infection were analyzed.Results 60 strains of CRPA were mainly from lower Respiratory tract secretions(30 strains, 50.0%) and urinary samples(10 strains, 16.7%),concentrating in intensive care unit(26 strains, 43.3%) and cardiovascular surgery(13 strains, 21.7%).A variety of factors may lead to the occurrence of CRPA infection through univariate analysis.Having used aminoglycosides(OR=0.339,95%CI:0.146~0.790,P=0.012) and used of more than 3 antimicrobials(OR=0.247,95%CI:0.068~0.898,P=0.034) may be important risk factors for CRPA infection.The mortality rate of CRPA infection was higher than CSPA infection(P=0.003).Conclusion The infection caused by CRPA seriously affects the prognosis of patients.Active measures should be taken to improve the control ability of CRPA.At the same time, it is necessary to strengthen the management of various antibiotics and environmental hygiene monitoring to reduce the occurrence and prevalence of CRPA infection in hospital.
