C反应蛋白、D-二聚体联合超敏肌钙蛋白I在急性主动脉夹层早期诊断中的应用价值

【摘要】 目的 评估C反应蛋白(CRP)、D-二聚体联合超敏肌钙蛋白I对急性主动脉夹层早期的诊断价值。方法 选取30例急性主动脉夹层患者作为急性主动脉夹层组,另选取30例急性心肌梗死患者作为急性心肌梗死组。比较两组患者C反应蛋白、D-二聚体水平及超敏肌钙蛋白I升高情况。结果 急性主动脉夹层组患者C反应蛋白(45.7±15.6)mg/L、D-二聚体(4.25±0.58)mg/L均高于急性心肌梗死组的(12.7±3.8)、(0.47±0.07)mg/L,超敏肌钙蛋白I升高占比10%低于急性心肌梗死组的100%,差异具有统计学意义(P<0.05)。结论 主动脉夹层患者早期与急性心肌梗死患者入院时C反应蛋白、D-二聚体、超敏肌钙蛋白I水平有明显差异,三者联合检查可以用于急性主动脉夹层的早期诊断,值得临床推广。

【Abstract】 Objective To evaluate the value of C-reactive protein(CRP) and D-dimer combined with hypersensitive cardiac troponin I in early diagnosis of acute aortic dissection. Methods A total of 30 patients with acute aortic dissection were selected as the aortic dissection group, and 30 patients with acute myocardial infarction were selected as the acute myocardial infarction group. The C-reactive protein, D-dimer level and increase of hypersensitive cardiac troponin I were compared between the two groups. Results The C-reactive protein of(45.7±15.6) mg/L and D-dimer of(4.25±0.58) mg/L in aortic dissection group were higher than those of(12.7±3.8) and(0.47±0.07) mg/L in acute myocardial infarction group; the increase rate of hypersensitive cardiac troponin I of 10% in aortic dissection group was lower than that of 100% in acute myocardial infarction group; the differences were statistically significant(P<0.05). Conclusion The levels of C-reactive protein,D-dimer and hypersensitive cardiac troponin I in the aortic dissection patients are significantly different from those in the acute myocardial infarction patients at early admission. The combined examination of the three can be used for the early diagnosis of acute aortic dissection, and is worthy of clinical promotion.
