新型冠状病毒感染与妊娠
Coronavirus disease 2019 and pregnancy
【摘要】 新型冠状病毒感染(coronavirus disease 2019,COVID-19)是由新型冠状病毒(severe acute respiratory syndrome coronavirus 2,SARS-CoV-2)引起的一种新型高度传染性疾病。COVID-19大流行以来,越来越多的研究致力于探讨COVID-19给患者带来的影响。孕妇由于妊娠期间发生免疫和生理适应性重塑,因此其健康备受关注。相关数据支持妊娠是与COVID-19相关严重疾病的危险因素,美国疾病预防控制中心COVID-19监测系统报告,与非妊娠妇女比较,孕妇更有可能被送入重症监护病房,且更需要有创通气和体外膜肺氧合,并且死亡率显著高于非妊娠妇女。越来越多的证据表明,妊娠期感染新型冠状病毒,特别是患有严重COVID-19疾病的孕妇,与许多不良妊娠结局有关,如先兆子痫、早产、流产、胎儿生长受限等。除了COVID-19对妊娠结局的直接影响外,还有证据表明,COVID-19大流行及其对医疗系统的影响也对妊娠结局产生了不利影响,例如死产和孕妇死亡率增加。本文对妊娠期间感染新型冠状病毒孕妇的临床特征、妊娠结局以及相关预防和治疗等做一综述,希望为COVID-19孕妇的治疗和管理等提供依据。
【Abstract】 Coronavirus disease 2019(COVID-19) is an emerging highly infectious disease caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2). Since the outbreak of the COVID-19 pandemic, more and more researches have explored the effects of COVID-19 infection on individual patients, among whom pregnant women in particular attracted high attention because of the immune and physiological adaptation they experience during pregnancy. Relevant studies have shown that pregnancy is a risk factor for severe complications associated with COVID-19. According to reports from the United States Centers for Disease Control and Prevention COVID-19surveillance system, compared with non-pregnant women, pregnant women are more likely to be admitted to an intensive care unit, require invasive ventilation, require extracorporeal membrane oxygenation, and die from COVID-19. Increasing evidences show that COVID-19 infection during pregnancy, especially severe COVID-19, is associated with several adverse pregnancy outcomes, such as pre-eclampsia, preterm birth, miscarriage, and fetal growth restriction. In addition to those direct harms, it has been reported that the pandemic and its impact on healthcare systems also impose adverse effects on pregnancy outcomes indirectly, such as increased stillbirths and maternal mortality. This article summarizes the clinical manifestations and outcomes of women infected with COVID-19 during pregnancy, and the prevention and treatment for this population, so as to provide evidence for the treatment and management of pregnant women infected with COVID-19.
