【摘要】 目的：比较铥激光前列腺剜除术（ThuLEP）与经尿道前列腺电切术（TURP）治疗良性前列腺增生（BPH）患者的效果。方法：选取2019年10月至2022年10月该院收治的126例BPH患者进行前瞻性研究，按照随机数字表法分为观察组与对照组各63例。对照组给予TURP治疗，观察组给予ThuLEP治疗，比较两组手术指标水平、尿动力学指标水平、性激素水平、病情严重程度分级和并发症发生率。结果：观察组组织切除量多于对照组，术后膀胱冲洗时间、手术时间、尿管留置时间短于对照组，差异有统计学意义（P<0.05）；两组术中出血量比较，差异无统计学意义（P>0.05）；术后3个月，两组最大尿道闭合压、最大尿流率、残余尿量、血清睾酮水平、血清雌二醇水平、病情严重程度分级以及并发症发生率比较，差异无统计学意义（P>0.05）。结论：ThuLEP治疗BPH可缩短术后膀胱冲洗时间、手术时间、尿管留置时间，提高组织切除量，效果优于TURP治疗。更多还原

【Abstract】 Objective: To compare efficacy of Thulium laser enucleation of the prostate(ThuLEP) and transurethral resection of the prostate(TURP) in treatment of benign prostatic hyperplasia(BPH) patients. Methods: A prospective study was conducted on 126 patients with BPH admitted to the hospital from October 2019 to October 2022. They were divided into observation group and control group according to the random number table method, 63 cases in each. The control group was treated with TURP, while the observation group was treated with ThuLEP. The surgical index levels, the urodynamic index level, the sex hormone level, the disease severity classification, and the incidence of complications were compared between the two groups. Results: The amount of tissue resection in the observation group was more than that in the control group; the postoperative bladder irrigation time, the operation time and the catheter indwelling time were shorter than those in the control group; and the differences were statistically significant(P<0.05). There was no significant difference in the intraoperative blood loss between the two groups(P>0.05). 3 months after the surgery, there were no significant differences in the maximum urethral closure pressure, the maximum urinary flow rate, the residual urine volume,the serum testosterone level, the serum estradiol level, the disease severity classification and the incidence of complications between the two groups(P>0.05). Conclusions: ThuLEP in the treatment of the BPH can shorten the postoperative bladder irrigation time, the operation time, the catheter indwelling time and the increase the amount of tissue resection. Moreover, it is superior to TURP treatment.更多还原
