【摘要】 目的 探讨雷公藤多苷(TWP)联合小剂量糖皮质激素(ICS)对于原发性肾小球肾炎(PGN)的治疗效果.方法 将118例PGN患者随机分为观察组、常规组，每组59例，观察组患者采取TWP联合ICS(小剂量)治疗；常规组患者采取ICS单纯治疗，比较两组的总有效率、临床指标、肾功能指标、免疫功能指标、炎性因子、不良反应率.结果 观察组的总有效率高于常规组(P<0.05),观察组的临床指标优于常规组(P<0.05).治疗前，两组患者肾功能指标、免疫功能指标、炎性因子水平比较差异无统计学意义(P>0.05).治疗8周后，观察组的24 h尿蛋白(24 h UP)、血肌酐(Scr)、尿素氮(BUN)小于常规组，观察组的β2微球蛋白(β2-MG)、尿素清除率(URR)与血内生肌酐清除率(Ccr)大于常规组(P<0.05).观察组的免疫功能指标大于常规组，观察组的炎性因子水平低于常规组(P<0.05).观察组的不良反应率低于常规组(P<0.05).结论 PGN患者实行TWP联合ICS(小剂量)治疗能够增强疗效，减轻患者的临床表现，改善其肾功能与免疫功能，缓解机体炎症反应，且用药副作用较少.更多还原

【Abstract】 Objective To select the therapeutic effect of Tripterygium wilfordii polyglycoside(TWP) combined with low dose glucocorticoid(ICS) on primary glomerulonephritis(PGN).Method 118 patients with PGN were randomly divided into observation group and conventional group.59 patients in observation group were treated with TWP and ICS(low dose);In the conventional group, 59 cases were treated with ICS alone, and the total effective rate, clinical indicators, renal function indicators, immune function indicators, inflammatory factors and adverse reaction rate were compared.Results The total effective rate of the observation group was higher than that of the conventional group(P<0.05).Before treatment, there was no statistically significant difference in renal function index, immune function index and inflammatory factor level between two groups(P>0.05).After 8 weeks of treatment, the 24-hour urine protein(24 h UP),serum creatinine(Scr),and urea nitrogen(BUN) in the obser-vation group were lower than those in the conventional group β2 Microglobulin(β2-MG),urea clearance rate(URR) and blood creatinine clearance rate(Ccr) were higher than those in the conventional group(P<0.05).The immune function index of the observation group was higher than that of the conventional group, and the inflammatory factor level of the observation group was lower than that of the conventional group(P<0.05).The adve-rse reaction rate of the observation group was lower than that of the conventional group(P<0.05).Conclusion The treatment of TWP combined with ICS(low dose) in PGN patients can enhance the curative effect, alleviate the clinical manifestations, improve the renal function and immune function, alleviate the inflammatory reaction of the body, and have fewer side effects.更多还原
