NT-proBNP水平在老年血液透析患者干体重评估中的应用价值
The Value of NT-proBNP Level in the Assessment of Dry Weight in Elderly Hemodialysis Patients
【摘要】 目的 探讨N末端B型脑钠肽前体(NT-proBNP)在老年血液透析患者干体重评估中的应用价值。方法 选取2021年1月-2022年1月于本院接受血液透析的老年患者及体检健康者,分别纳入观察组和对照组,每组30例。观察组经过14天逐渐透析脱水下调体重达到临床干体重。测定观察组下调体重前后和对照组的NT-proBNP水平、心胸比值及体重,并观察临床表现。结果 观察组体重下调前后的NT-proBNP、体重及心胸比值差异均具有显著性意义(P<0.05);观察组干体重下调后与对照组的NT-proBNP水平有统计学差异(P<0.05),在体重、心胸比值差异的比较差异无统计意义(P>0.05)。血液透析患者体重下调前后NTproBNP水平差与心胸比值差呈强相关关系(P<0.05)。观察组体重下调后,患者的腹部饱胀和恶心反应、胸闷和气促反应、高血压反应、水肿反应症状消失或是得到明显缓解。结论 老年血液透析中,NT-proBNP水平可作为干体重评估的敏感指标,具有较高的便捷性和可靠性,可用于临床推广应用。
【Abstract】 Objective To investigate the clinical value of NT-proBNP in dry weight assessment in elderly hemodialysis patients. Methods From January 2021 to January 2022, elderly patients and healthy patients who underwent hemodialysis in our hospital were enrolled into the treatment group and the control group, with 30 cases in each group. After 14 days of dehydration, the treatment group lowered their body weight to dry weight. The NT-proBNP level, cardiothoracic ratio and body weight,and the adverse effect of each groups were observed. Results The differences in NT-proBNP, body weight and cardiothoracic ratio before and after weight reduction in the treatment group were of significant significance(P<0.05), there was a significant difference between the NT-proBNP level of dry weight reduction and the control group after the reduction of dry body weight in the observation group(P<0.05), and there was no statistical difference in the comparison between the difference between body weight and cardiothoracic ratio(P>0.05). The difference between NT-proBNP levels before and after weight reduction in hemodialysis patients was strongly correlated with the difference in cardiothoracic ratio(P<0.05). After the weight reduction, the patient’s adverse effects disappear or are significantly relieved. Conclusion In elderly hemodialysis, NT-proBNP level can be used as a sensitive index for dry body weight assessment, with convenience and reliability, and can be promoted.
