基于气道内雾化吸入的护理模式在呼吸机辅助治疗小儿重症肺炎合并呼吸衰竭中的应用效果
Application effect of nursing mode based on airway atomization inhalation in ventilator assisted treatment of severe pneumonia in children with respiratory failure
【摘要】 目的 探究呼吸机辅助治疗小儿重症肺炎合并呼吸衰竭中应用基于气道内雾化吸入护理模式的效果。方法 选取2021年1月至2022年7月赣州市妇幼保健院儿童重症监护科收治的70例重症肺炎合并呼吸衰竭患儿作为研究对象,按照随机数字表法分为对照组(35例)和观察组(35例)。对照组患儿采取常规护理方法,观察组患儿在对照组常规护理基础上采用基于气道内雾化吸入的护理模式。对比两组患儿的治疗时间指标(呼吸机辅助治疗时间、肺啰音消失时间、呼吸困难缓解时间、住院时间),生命体征[血氧饱和度(SpO2)、氧分压(PaO2)],肺功能指标[最大呼气压(MEP)、最大吸气压(MIP)、最大呼气中端流量(MMF)]、不良事件发生情况以及出院时、随访1、3个月的生活质量。结果 观察组的呼吸机辅助治疗时间、肺啰音消失时间、呼吸困难缓解时间、住院时间短于对照组,差异有统计学意义(P<0.05)。出院时,观察组的SpO2、PaO2高于对照组,差异有统计学意义(P<0.05);观察组MEP、MMF高于对照组,MIP低于对照组,差异有统计学意义(P<0.05)。对照组护理后不良事件总发生率高于观察组,差异有统计学意义(P<0.05)。出院时观察组的生活质量高于对照组,差异有统计学意义(P<0.05),随访1、3个月观察组的生活质量数据均高于对照组,差异有统计学意义(P<0.05)。结论 呼吸机辅助治疗小儿重症肺炎合并呼吸衰竭应用基于气道内雾化吸入护理模式的效果是积极的,能够有效缩短患儿的呼吸机辅助治疗时间,稳定生命体征,提高肺功能指标,减少不良事件的发生情况,且有助于患儿的治疗预后,值得在临床进行广泛的推进和使用。
【Abstract】 Objective To explore the effect of nursing mode based on airway atomization inhalation in the assisted treatment of severe pneumonia complicated with respiratory failure in children with ventilator. Methods A total of 70 children with severe pneumonia complicated with respiratory failure admitted to the Children’s Intensive Care Department of Ganzhou Maternal and Child Health Hospital from January 2021 to July 2022 were selected as the research subjects. They were divided into a control group(35 cases) and an observation group(35 cases) using the random number table method. The control group of children received routine nursing care, while the observation group of children received a nursing model based on airway atomization inhalation on the basis of routine nursing care in the control group. The treatment time indicators(ventilator assisted treatment time, lung rale disappearance time, dyspnea relief time, hospital stay), vital signs(oxygen saturation[SpO2], partial oxygen pressure [PaO2]), lung function indicators(maximum expiratory pressure [MEP], maximum inspiratory pressure [MIP], maximum midexpiratory flow [MMF]), the occurrence of adverse events and the quality of life at discharge,1-month follow-up, and 3-month follow-up between the two group were compared. Results The time of ventilator assisted treatment, disappearance time of pulmonary rales, relief time of dyspnea and hospital stay in the observation group were shorter than those in the control group, and the differences were statistically significant(P<0.05). At discharge, SpO2 and PaO2 in observation group were higher than those in control group, and the differences were statistically significant(P<0.05). MEP and MMF in observation group were higher than those in control group, and MIP was lower than that in control group, the differences were statistically significant(P<0.05). The total incidence of adverse events after nursing in control group was higher than that in observation group, and the difference was statistically significant(P<0.05). The quality of life in the observation group was higher than that in the control group at discharge, the difference was statistically significant(P<0.05), and the quality of life data in the observation group was higher than that in the control group at 1 and 3 months follow-up, the difference was statistically significant(P<0.05). Conclusions The effect of ventilator assisted treatment of severe pneumonia complicated with respiratory failure based on endotracheal atomization inhalation nursing mode is positive, which can effectively shorten the time of ventilator assisted treatment, stabilize vital signs, improve lung function indicators, reduce the occurrence of adverse events, and contribute to the treatment prognosis of children, worthy of extensive clinical promotion and use.
