【摘要】 目的 探讨社区安宁疗护在终末期癌症患者的应用效果。方法选取2021年10月至2022年9月东莞市人民医院肿瘤中心收治并转介到东莞市大朗镇社区卫生服务中心进行社区服务的50例终末期癌症患者作为研究对象，按照随机数字表法分为对照组（25例）与研究组（25例）。对照组采用常规的护理策略，研究组在对照组护理干预方式基础上将安宁疗护工作模式融入其中。比较两组患者护理干预前后的疼痛情况评分[疼痛数字分级法（NRS）]、基本生活活动能力（BADL）评定量表、心理状态[抑郁自评量表（SDS）和焦虑自评量表（SAS）]、心理韧性水平[心理韧性量表（CD-RISC）]，比较两组患者的护理干预满意度。结果 干预前，两组患者的NRS、BADL、SDS、SAS、心理韧性评分比较，差异无统计学意义（P>0.05）；干预后，两组患者的NRS、SDS、SAS评分均低于本组干预前，BADL、心理韧性评分均高于本组干预前，差异有统计学意义（P<0.05）。研究组患者干预后的NRS、SDS、SAS评分均低于对照组，BADL、心理韧性评分均高于对照组，差异有统计学意义（P<0.05）。研究组患者护理满意度各个维度的评分均高于对照组，差异有统计学意义（P<0.05）。结论 在终末期癌症患者的护理过程中，使用社区安宁疗护的策略，可有效缓解患者疼痛程度，提高生活质量、情绪反应和心理韧性水平，提高患者满意度，对临床工作具有重要应用价值。更多还原

【Abstract】 Objective To explore the effect of community hospice care in end-stage cancer patients. Methods A total of 50patients with end-stage cancer admitted to Cancer Center of Dongguan People’s Hospital from October 2021 to September2022 and referred to the Dongguan Dalang Town Community Health Service Center for community service were selected as the research objects, and they were divided into the control group(25 cases) and the research group(25 cases) according to random number table method. The control group adopted conventional nursing strategies. On the basis of nursing intervention in the control group, the research group integrated the hospice care mode into it. The pain score(numerical rating scale [NRS]), basic activity of daily living(BADL) rating scale, psychological state(self-rating depression scale [SDS]and self-rating anxiety scale [SAS]) and psychological resilience level(connor-davidson resilience scale [CD-RISC]) of the two groups of patients before and after nursing intervention were compared. And the nursing intervention satisfaction of two groups of patients were compared. Results There were no significant differences in NRS score, BADL score, SDS score, SAS score and psychological resilience score between the two groups before nursing intervention(P>0.05). After the nursing intervention, the NRS score, SDS score and SAS score of the two groups were lower than those before the nursing intervention, while the BADL score and psychological resilience score of the two groups were higher than those before the nursing intervention, with statistically significant differences(P<0.05). The NRS score, SDS score and SAS score of patients in the research group after nursing intervention were lower than those in the control group, while the BADL score and psychological resilience score were higher than those in the control group, with statistically significant differences(P<0.05).The scores of patients in the research group in all dimensions of nursing intervention satisfaction were higher than those in the control group, with statistically significant differences(P<0.05). Conclusion In the nursing process of end-stage cancer patients, the use of community hospice care strategies can effectively alleviate the pain level of patients, improve the quality of life, emotional response and psychological resilience, and improve patient satisfaction, which has important application value for clinical work.更多还原
