机器人辅助置入骶髂螺钉治疗不稳定骨盆骨折的效果
Effect of robot-assisted percutaneous sacroiliac screw placement in the treatment of unstable pelvic fractures
【摘要】 目的 探讨骨科机器人辅助下经皮置入骶髂螺钉的临床效果。方法 回顾性分析2017年9月至2020年9月山东大学第二医院收治的87例Tile B型及C型骨盆骨折患者临床资料,根据手术方式分为观察组(n=43)和对照组(n=44)。观察组采用机器人辅助下置入骶髂螺钉,对照组采用传统方法经皮置入骶髂螺钉。比较两组患者的手术时间、术中出血量及透视次数,随访统计患者术后Majeed评分、Matta评分及并发症发生情况。结果 观察组的手术时间短于对照组,术中出血量及透视次数少于对照组,差异有统计学意义(P<0.05)。术后随访,观察组的Majeed评分优良率高于对照组,Majeed评分分级优于对照组,差异有统计学意义(P<0.05)。两组患者的Matta评分优良率比较,差异无统计学意义(P>0.05),但观察组的Matta评分分级优于对照组,差异有统计学意义(P<0.05)。两组患者的并发症总发生率比较,差异无统计学意义(P>0.05)。结论 骨科机器人辅助下经皮置入骶髂螺钉在缩短手术时间、减少术中出血量,降低透视次数方面具有良好的临床效果。
【Abstract】 Objective To investigate the clinical effect of percutaneous implantation of sacroiliac screw assisted by orthopedic robot. Methods The clinical data of 87 patients with Tile type B and type C pelvic fractures admitted to the Second Hospital of Shandong University from September 2017 to September 2020 were retrospectively analyzed, and they were divided into observation group(n=43) and control group(n=44) according to the surgical method. The sacroiliac screw was inserted in the observation group with the assistance of robot, and the sacroiliac screw was inserted percutaneous in the control group with the traditional method. The operation time, intraoperative blood loss and fluoroscopy times of the two groups were compared, and the postoperative Majeed score, Matta score and the incidence of complications were followed up.Results The operation time of the observation group was shorter than that of the control group, and the intraoperative blood loss and fluoroscopy times were less than that of the control group, the difference was statistically significant(P<0.05). In postoperative follow-up, the excellent and good rate Majeed score of observation group was higher than that of control group,and the Majeed score grading was better than that of control group, with statistical significance(P<0.05). There was no statistically significant difference in the excellent and good rate Matta score of the two groups(P>0.05), but the Matta score of the observation group was better than that of the control group, and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of complications between two groups(P >0.05). Conclusion Robot-assisted sacroiliac screw placement has a good clinical effect in shortening the operation time, reducing intraoperative blood loss and reducing the number of fluoroscopy.
