绝经后2型糖尿病患者血清同型半胱氨酸与肌少症相关性的研究
Correlation between serum homocysteine and sarcopenia in postmenopausal women with type 2 diabetes mellitus
【摘要】 目的 探讨绝经后女性T2DM患者血清同型半胱氨酸(Hcy)与肌少症的相关性。方法 选取2020年9月至2022年9月于秦皇岛市第一医院内分泌科住院治疗的绝经后T2DM患者169例,分为单纯T2DM组(n=113)和合并肌少症组(SP,n=56),比较两组一般资料及生化指标,分析肌少症的危险因素。结果 绝经后T2DM患者肌少症患病率为33%。SP组年龄、FPG、Hcy高于T2DM组(P<0.05),BMI、血红蛋白(Hb)低于T2DM组(P<0.05)。Hcy三分位组(Q1≤10.24μmol/L,Q2 10.25~12.68μmol/L,Q3≥12.69μmol/L)SUA、Scr依次升高(P<0.05),握力、步速依次降低(P<0.05)。Spearman相关分析显示,Hcy与年龄、血尿酸(SUA)、血肌酐呈正相关(P<0.05),与握力、步速、Hb呈负相关(P<0.05)。Logistic回归分析显示,年龄、BMI、SUA、Hcy是绝经后T2DM患者合并SP的影响因素。结论 绝经后T2DM患者Hcy升高与肌少症密切相关,与肌肉功能的相关性更显著。
【Abstract】 Objective To investigate the correlation between homocysteine(Hcy)level and sarcopenia(SP)in postmenopausal women with type 2 diabetes mellitus(T2DM).Methods A total of 169postmenopausal female with T2DM who were hospitalized in the department of endocrinology were enrolled from September 2020 to September 2022. According to the diagnostic criteria of 2019 Asian working group on sarcopenia,all the subjects were divided into sarcopenia group(SP,n=56)and T2DM group(T2DM,n=113). The general data and laboratory indicators were collected and compared.Results The incidence of SP in postmenopausal T2DM patients was 33%. Compared with T2DM group,the levels of age,FPG and Hcy in SP group were higher(P<0. 05),while BMI and Hb were lower(P<0. 05). In the patients with higher Hcy(tertile group Q1≤10. 24 μmol/L,Q2 10. 25~12. 68 μmol/L,Q3≥12. 69 μmol/L),SUA and Scr increased sequentially(P<0. 05),grip strength and walking speed decreased sequentially(P<0. 05). Spearman correlation analysis showed that Hcy was positively correlated with age,SUA and Scr(P<0. 05),while negatively correlated with grip strength,stride speed and Hb(P<0. 05). Logistic regression analysis showed that age,BMI,SUA and Hcy were the influencing factors of SP in postmenopausal T2DM patients.Conclusion Increased Hcy in postmenopausal women with T2DM is associated with SP,and is more closely associated with muscle function.
